MRI Feathers of Hemangioblastoma of Central Nervous System

目的:通过分析中枢神经系统血管母细胞瘤的MRI表现,探讨MRI对中枢神经系统血管母细胞瘤的诊断价值。材料与方法:回顾性分析24例经手术病理证实为血管母细胞瘤的MRI表现。结果:本组24例患者18例单发,6例多发,共计36个病灶,小脑半球22个,小脑蚓部4个,小脑扁桃体2个,延髓2个,桥小脑角区2个,脊髓4个;MR表现囊性型22例,其中囊伴结节型21个,单纯囊肿型1个,实质型14个,单纯实性肿块型1例,肿块伴囊变1例,实性结节型12个,实性结节型1例单发,5例多发;肿瘤囊性部分呈长T1长T2信号,增强后无强化,实性部分或壁结节呈等长T1等长T2信号,增强均呈明显强化;6例患者行弥散加权成像(DWI),病灶囊性部分呈低信号影,实性部分呈等低信号影;2例Von-Hipple-Lindau综合征。结论:血管母细胞瘤的MRI增强表现具有一定的特征性,能为术前诊断提供可靠的诊断依据,结合DWI序列有助于鉴别诊断。

Purpose： By analyzing the MRI feature of hemangioblastoma of central nervous system to investigate its diagnostic value. Materials and Methods： Retrospective method is applied to analyze the data of MR images from 24 cases of Hemangioblastoma confirmed by operation and pathology. Results ： There are a total of 36 lesions in 24 patients among which 18 are single types of Hemangioblastoma , 6 are multiple types. 22 in the cerebellar hemisphere, 4 in cerebellar vermis, 2 in Chiari, 2 in medulla oblongata, 2 in eerebellopontine angle, and 4 in the spinal cord. MRI characteristics of the lesions indi- cate 22 cystic types, with 21 nodular of cystic types and 1 pore cystic type, and 14 solid types , with 1 pure solid type, 1 cyst- ic mass, and 12 solid nodules. Among the solid nodules, 1 case is single types and 5 are multiple types. The cystic regions of the mass indicate long T1 and T2 signals, and no enhancement was found. While the solid type and mural nodules indicate e- ven long T1 and T2 signals, and significant enhancement was observed. The images of 6 patients were conducted with DWI （ Diffusion Weighted Images） , with cystic part of lesions demonstrating low signal while the solid part equal low signal. Anoth- er 2 patients were Von-Hipple-Lindau syndrome. Conclusion： Enhanced Hemangioblastoma MRI images demonstrate certain characteristics, which can provide reliable diagnosis to preoperation diagnosis. Combined with DWI sequence it can help to confirm the diagnosis.
