宁波市社区中老年人糖尿病前期人群筛查与血脂特征分析

Screening of middle-aged community residents with pre-diabetes and analysis of lipid characteristics in Ningbo

目的了解社区中老年人中PDM人群的血糖与血脂特征,探讨多指标联合筛查的可行性。方法 9 946名社区中老年人用FPG、2 h PG筛查PDM;选择其中1 048名PDM筛查血脂（TG、CHO、HDL、LDL）、糖化血红蛋白和胰岛素水平,分析其特征。结果 9 946名中老年人,不同性别间FPG、2 h PG水平差异无统计学意义（χ2=1.53,P=0.217）,随年龄增长有增高趋势,差异有统计学意义（χ2=138.217,P〈0.01）。2 h PG检测DM漏检率为13.13%（91/693）;PDM漏检率为13.95%（277/1 986）。TG、CHO、HBA1C、HDL、INS、LDL各项都存在超标人群,以CHO最高。结论社区中老年人有大量的PDM和DM隐匿其中,多无临床症状,检出率高于全国平均水平;血脂、糖化血红蛋白与血糖一样,在PDM人群中常伴随发生,联合检测能早期发现PDM及并发症,对监控社区中老年人中DM主要危险因素与血糖稳定有着重要意义,具有推广与应用价值。

Objective To investigate the characteristics of blood glucose and lipid in the PDM population of elderly communities residents,so as to explore the feasibility of multi- index combined screening. Methods 9 946 middle- aged and elderly residents in communities were screened for PDM by blood glucose index（ FPG,2h PG）,while 1 048 PDM were screened by blood lipids（ TG,CHO,HDL,LDL）,glycosylated hemoglobin and insulin,analyzing the characteristics. Results There was no statistical significance on the difference in the levels of FPG and 2 h PG between different genders（ χ2 1. 53,P = 0. 217）.However,the levels of FPG and 2 h PG increased gradually with ages,with the difference statistically significant（ χ2= 138. 217,P 〈 0. 01）. The missing rate of 2 h PG in the detection of DM was 13. 13%（ 91/693）; that of PDM was 13. 95%（ 277/1 986）. There were TG,CHO,Hb A1 C,HDL,INS,LDL exceeding crowds,with CHO the highest. Conclusion Many PDM and DM people are insidious among the elderly residents in the communities. Most of them have no clinical symptom. The detection rate is above the national average level. Blood lipids,glycosylated hemoglobin and glucose often accompany in the PDM population. Combined assay can detect PDM population and its complications in early stage,showing significance in monitoring the main risk factors and the stability of blood glucose of DM people in the elderly population in the communities,having its promotion and application value.
