广州市海珠区“四位一体”艾滋病病例综合管理模式效果评价

Effect of “Four Measures” comprehensive case management for HIV/AIDS in Haizhu District of Guangzhou City

目的评价广州市海珠区"四位一体"艾滋病病例综合管理模式对艾滋病病毒感染者和艾滋病病人(HIV/AIDS病例)的高危行为、生活质量与社会支持的影响。方法调查广州市海珠区艾滋病病例综合管理模式运行后292例艾滋病病例的行为、生活质量和社会支持情况,并与模式运行之初的结果进行比较分析。结果与获知感染前相比,72.9%(204/280)的病例自诉性伴数减少,75.6%(118/156)的病例安全套使用频率增加;共用注射器吸毒的频率减少;模式运行后,生活质量4个维度生理、心理、社会关系和环境的平均得分分别为(59.8±16.5)、(56.2±15.4)、(54.8±15.4)分、(54.2±14.6)分,其中心理和环境得分均高于体系运行之前(P<0.05);社会支持总分为(28.3±8.2)分,高于模式运行之初的(25.5±9.0)(t=2.843,P<0.01)。结论广州市海珠区艾滋病病例综合管理模式对降低高危行为,增加社会支持程度及提高生活质量发挥了积极作用。

Objective To evaluate the effect of human immunodeficiency virus/acquired immunodeficiency syndrome （HIV/ AIDS） comprehensive case management in Guangzhou City for improvements of high - risk behavior, quality of life and social sUpport among people living with HIV/AIDS. Methods Totally 292 HIV/AIDS cases were interviewed face - to - face to obtain information on high risk behavior changed before and after HIV infection. The qtiality of life, social support of the subjects were compared With those before the implementation of the mode. Results After the notification, 72.9% （204/280） of HIV/AIDS had lessen number of sexual partner, 75.6% （118/156）bad increased condom use, 100.0% （7/7） had decreased frequency of sharing of syringe. The average scores of quality of life for physical, psychological, social relationship, and environment were （59. 8 ± 16. 5 ） , （56. 2± 15.4 ） , （54. 8 ± 15.4 ） and （54. 2 ± 14. 6 ）. The average scores of psychological and environment were higher than before mode operation （ P 〈 0. 05 ）. The total score of social support （ 28.3± 8.2 ） was higher than before （ 25.5±9. 0 ） （ t = 2. 843, P 〈 0. 01 ）. Conclusion The HIV/AIDS comprehensive case management in Haizhu District of Guangzhou plays a positive role in reducing high - risk behaviors, increasing the degree of social support and improving the quality of life of people living with HIV/AIDS.
