摘要
目的:探讨门冬胰岛素与生物合成人胰岛素治疗妊娠期糖尿病的临床效果。方法:选取2014年2月—2015年9月齐齐哈尔医学院附属第一医院收治的妊娠期糖尿病患者92例作为研究对象,以随机数字表法分为观察组和对照组各46例。观察组患者给予门冬胰岛素+中效胰岛素治疗,对照组患者给予中效胰岛素+生物合成人胰岛素治疗。比较2组患者血糖变化情况、胰岛素的有效性、分娩结局和母婴不良结局发生情况。结果:治疗后3 d,观察组早餐后2 h血糖明显低于对照组,差异有统计学意义(P<0.05);治疗5 d后,2组患者空腹血糖与糖化血红蛋白均明显低于治疗前,差异有统计学意义(P<0.05);观察组患者满意度明显高于对照组,餐前低血糖事件发生率明显低于对照组,差异均有统计学意义(P<0.05);2组患者的分娩结局、母婴不良结局发生率的差异无统计学意义(P>0.05)。结论:门冬胰岛素治疗妊娠期糖尿病,效果确切,安全性好。
OBJECTIVE: To probe into the clinical effects of insulin and biosynthetic human insulin in treatment of patients with gestational diabetes mellitus. METHODS: 92 patients with gestational diabetes mellitus admitted into the First Affiliated Hospital of Qiqihar Medical College,from Feb. 2014 to Sept. 2015 were selected to be divided into observation group and control group via the random number table,with 46 cases in each. The observation group were given insulin aspart combined with middle acting insulin,while the control group were treated with middle acting insulin combined with biosynthetic human insulin. The changes of blood glucose, insulin availability, delivery outcomes,incidence of maternal and neonatal outcomes of two group were compared. RESULTS: After treatment of3 d,the blood glucose of 2 h after breakfast in observation group was significantly lower than that of control group,with statistically significant difference( P〈0. 05). After treatment of 5 d,the fasting blood glucose and glycosylated hemoglobin of two groups were lower than that of before treatment,with statistically significant difference( P〈0. 05).The satisfaction of observation group was significantly higher than that of control group,while the incidence of hypoglycemia event before the meal was lower than that of control group,with statistically significant difference( P〈0. 05). There was no statistically significant difference between two groups in delivery outcomes,incidence of maternaland neonatal outcomes( P〈0. 05). CONCLUSIONS: The effects of insulin aspart in treatment of gestational diabetes mellitus is remarkable with high safety.
