摘要
目的通过分析慢性阻塞性肺病患者肺动脉压与氨基末端脑钠肽前体(NT-pro BNP)水平的关系,探讨肺动脉高压发病机制及治疗。方法采集所有入选患者的一般情况登记。入院后抽空腹血查血常规、血糖、血脂、凝血功能、血气分析等指标。行超声心动图、心电图、胸片或胸部CT、肺功能检查。同时对入选患者进行心、肺评估。对比肺动脉高压组及肺动脉正常组患者氨基末端脑钠肽前体(NT-pro BNP)的水平变化。对肺动脉高压组在常规治疗基础上予贝前列腺素钠治疗,对比治疗前后患者心、肺功能的变化。结果慢性阻塞性肺疾病肺动脉高压组NT-pro BNP显著高于肺动脉压正常组。予贝前列腺素钠治疗前后患者的、6MWT、Brog呼吸困难指数明显改善。结论肺动脉高压是慢性阻塞性肺疾病常见合并症之一,针对肺动脉高压进行治疗,可提高患者生活质量,使患者获益。
Objective To analyze the relationship between pulmonary arterial pressures and the amino terminal pro-brain natriuretic peptide (NT-proBNP) levels of chronic obstructive pulmonary disease in patients,and to explore the pathogenesis and treatment of pulmonary hypertension. Methods The collection of all patients in general registration.Routine blood test, blood sugar,blood lipids,blood coagulation,blood gas analysis and other indicators were check by fasting blood after admission. Echocardiography,ECG,chest X-ray or chest CT and pulmonary function tests were also checked.At the same time, heart and lung of the selected patients were assessed.The level of amino-terminal pro-brain natriuretic peptide (NT-proBNP) of pulmonary hypertension group and normal control group of patients with pulmonary were compared.Patients in pulmonary hypertension group were treated with sodium beta prostaglandin on the basis of conventional therapy,heart and lung function before and after the treatment were compared. Results NT-proBNP of pulmonary chronic obstructive pulmonary disease pulmonary hypertension group was significantly higher than that of Normal pulmonary artery pressure group.Sodium lung function before and after treatment to beraprost patients,6MWT,Brog dyspnea index improved significantly. Conclusion Pulmonary hypertension is a common complication of chronic obstructive pulmonary disease.Good treatment can benefit and improve the quality of life of patients.
