股骨远端LISS钢板倒置治疗复杂股骨粗隆间骨折的疗效分析

Curative effect analysis of LISS plate inversion of distal femur in treatment of complex intertrochanteric fracture of femur

目的 探讨股骨远端LISS钢板倒置治疗复杂股骨粗隆间骨折的临床疗效。方法 选取2013年2月~2015年2月我院就诊的复杂股骨粗隆间骨折患者65例,所有患者随机分为观察组33例及对照组32例。对照组采用动力髋螺钉（DHS）治疗,观察组采用股骨远端LISS钢板倒置治疗。观察两组各自的切口长度、手术出血量、手术用时,术后定期随访记录骨折临床愈合时间,髋关节功能Harris评分及并发症情况。结果 观察组患者的切口长度、手术时间、手术出血量、骨折临床愈合时间结明显少于对照组,两组比较差异有统计学意义（P均〈0.05）。观察组患者的优良率为90.91%。明显高于对照组的75.00%,两组比较差异有统计学意义（P〈0.05）。观察组患者并发症发生率为6.06%,明显低于对照组的15.63%,两组比较差异无统计学意义（P均〈0.05）。结论 股骨远端LISS钢板倒置治疗复杂股骨粗隆间骨折的临床疗效满意,可以明显降低不良反应发生率,值得临床推广应用。

Objective To explore the clinical effect of LISS plate inversion of distal femur in treatment of complex intertrochanteric fracture of femur. Methods 65 cases of patients with complex femoral intertrochanteric fracture cured in our hospital from February 2013 to February 2015 were selected and randomly divided into observation group with 33 cases and control group with 32 cases. Patients in control group were treated with dynamic hip screw（DHS）, and patients in observation group were treated with LISS plate inversion of distal femur. The length of the incision, the amount of bleeding, and the operation time of the two groups were observed. The clinical healing time, hip function Harris score and complications were postoperatively follow-up recorded. Results Length of incision, operation time, amount of bleeding, clinical healing time of fracture of observation group were significantly less than those of control group. The difference between the two groups was significant（P 〈 0.05）. The excellent and good rate of observation group was 90.91%, which was significantly higher than that of control group with 75%, and the difference was significant（P 〈 0.05）. The complication rate of observation group was 6.06%, which was significantly lower than that of control group with 15.63, and the difference was significant（P 〈 0.05）. Conclusion The clinical effect of LISS plate inversion of distal femur in treatment of complex intertrochanteric fracture of femur is satisfied. It can significantly reduce the incidence of adverse reactions, and it is worthy of clinical application.
