关节镜下常规清理联合去神经化治疗中晚期膝关节骨性关节炎中的临床效果

The effect of routine arthroscopic cleanup combined with denervation therapy on patients with advanced knee osteoarthritis

目的观察关节镜下常规清理联合去神经化治疗中晚期膝关节骨性关节炎中的临床效果以及安全性。方法将2013年3月至2016年3月期间80例中晚期膝关节OA病人随机分成对照组与观察组。对照组进行关节镜下常规清理治疗,观察组进行关节镜下常规清理联合去神经化治疗。比较两组的疼痛程度、术后切口愈合时间以及膝关节功能等。结果手术1个月后观察组的视觉模拟评分(VAS评分)明显的低于对照组,差异具有显著性(P<0.05);手术后1个月、3个月观察组的Lysholm评分明显高于对照组,差异均具有显著性(P<0.05);手术后3个月,观察组与对照组的HSS评分优良率分别为95.03%和80.0%,观察组的优良率明显高于对照组,差异具有显著性(P<0.05)。结论关节镜下常规清理联合去神经化治疗可有效缓解膝关节OA病人的疼痛程度,促进膝关节功能恢复,提高临床疗效,且安全性好。

Objective To study the effect and safety of routine arthroseopic cleanup combined with danervation therapy on advanced knee asteoarthritis. Methods From March 2013 and March 2016, 80 patients with advanced knee osteoarthritis in our hospital were randomly divided into control group and observation group. And cleanup routine arthroscopic treatment was used in control group, denervation therapy combined with cleanup routine arthroscopic were used in observation group. The pain vulgar, postoperative wound healing time, and knee function of two groups were compared. Results 1 month after surgery, VAS scores in observation group was significantly lower than that in the control group, the differ- ence was significant （ P 〈 0.05 ）. 1 and 3 months after surgery Lysholm score in the observation group was significantly higher than that in the control group, the difference was significant （ P 〈 0.05 ）. 3 months after surgery, excellent rate of HSS score in the observation group and control group were 95.03% and 80.0%, respectively. The excellent rate of the observation group was significantly higher than that of control group, the difference was significant （ P 〈 0.05）. Conclusion Routine arthmscopic cleanup combined with denervation therapy can effectively relieve the pain level, and promote the recovery of knee function, improve the efficacy of treatment for patients with advanced knee osteoarthritis.
