多普勒超声在囊性肾癌诊断和鉴别诊断中的价值

Value of diagnosis and differential diagnosis of ultrasonography in cystic renal cell carcinoma

目的探讨多普勒超声在囊性肾癌诊断与鉴别诊断中的价值。方法回顾性分析2014年4月至2016年6月收治并经手术及病理证实为囊性肾癌的56例患者60个病灶的声像图表现等术前超声诊断资料。结果囊性肾癌的声像图表现复杂多样,术前超声诊断符合率91.7%(55/60),单房囊肿型6例,多房囊肿型26例,单纯性囊肿癌变型3例,肾癌囊性坏死型25例;误诊为血管平滑肌脂肪瘤2例,误诊为多房性囊肿1例,误诊为囊肿合并出血1例,误诊为干酪空洞型肾结核1例。结论囊性肾癌具有特殊的超声诊断表现,应多注意同良性复杂性肾囊肿如多房性肾囊肿,肾囊肿囊壁钙化,出血性肾囊肿等的鉴别,为临床肾脏疾病的诊断和治疗提供重要的影像学依据。

Objective To estimate the value of diagnosis and differential diagnosis of ultrasonography in cystic renal cell carcinoma （CRCC） by the analysis of the sonographic character. Methods Sonographic features of 56 CRCC patients with 60 lesions affirmed by surgery and pathology from April 2014 to June 2016 were retrospectively analyzed. Results Sonographic features of CRCC were complex, sonographic di- agnostic accordance rate was 91.7% （55/60）. There were single cavity type 6 cases, multi cavity type 26 cases, simple cyst canceration type 3 eases, renal carcinoma cystic necrosis type 25 cases. 2 cases were misdiagnosed as simple multi - cysts, 1 case as renal angiomyolipoma, 1 case as cheese cavity type tuberculosis. Conclusion CRCC owns special ultrasoound charicteristic. In differential diagnosis, we should pay attention to the benign complex renal cyst, for example multilocular cyst of renal, renal cyst wall calcification, hemorrhagic renal cyst, et al. Uhrasonogra- phy can provide important imaging evidence for early diagnosis and correct treatment.
