宫颈癌患者HPV感染情况分析及其宫颈组织CD4^+、CD8^+的表达研究 被引量:11

Analysis of infection with HPV and a study of the expression of CD4^+ and CD8^+ in cervical secretions from patients with cervical cancer

摘要 目的调查宫颈癌患者HPV感染情况,检测宫颈组织及分泌物中CD4^+和CD8^+的表达情况,为宫颈癌的治疗及控制提供指导。方法收集宫颈癌患者宫颈分泌物及其脱落细胞,采用PCR进行HPV DNA分型;采用免疫组化法和流式细胞术检测宫颈组织及分泌物中CD4^+和CD8^+的表达。结果宫颈癌患者宫颈分泌物HPV16型、HPV52型、HPV58型扩增片段分别为152bp、438bp和445bp,HPV16、HPV52、HPV58、HPV33、HPV18、HPV6以及其他型HPV阳性率分别为42.63%、21.31%、16.94%、8.74%、4.92%、2.73%和8.74%,且单一类型HPV感染多于多重类型感染。采用免疫组化法检测宫颈癌患者宫颈分泌物CD4^+和CD8^+细胞,阳性率分别为30.60%和62.84%,CD4^+/CD8^+值<1的宫颈癌患者阳性率(70.49%)高于比值≥1的患者(29.51%)。结论宫颈癌患者感染的HPV以16型为主,单一感染高危型HPV可能更易导致宫颈癌发生,及时检测宫颈分泌物中CD4^+和CD8^+对于疾病监控及治疗具有重要意义。 Objectives To study infection with HPV in patients with cervical cancer and to monitor the expression of CD4^+and CD8^+cells in cervical secretions in order to guide treatment and control cervical cancer. Methods Cervical secretions and exfoliative cells were collected.HPV DNA was amplified with PCR and typed,and the expression of CCD4^+and CD8^+cells in cervical secretions was detected immunohistochemically. Results The amplified fragment of HPV16 was 152bp in length,the amplified fragment of HPV52 was 438bp in length,and the amplified fragment of HPV58was445 bp in length.The types of HPV infecting patients with cervical cancer were analyzed,and results indicated that patients tested positive for HPV16 at a rate of 42.63%,patients tested positive for HPV52 at a rate of 21.31%,patients tested positive for HPV58 at a rate of 16.94%,patients tested positive for HPV33 at a rate of 8.74%,patients tested positive for HPV18 at a rate of 4.92%,patients tested positive for HPV6 at a rate of 2.73%,and patients tested positive for some other type at a rate of 8.74%.More patients with cervical cancer tested positive for a single type of HPV than for multiple types.CD4^+cells were expressed in 30.60% of the patients with cervical cancer(56/183),and CD8^+cells were expressed in 62.84%.Patients with cervical cancer and CCD4^+/CD8^+ 〈1 tested positive for HPV at a higher rate(70.49%)than patients with CCD4^+/CD8^+≥1. Conclusion The type of HPV infecting patients with cervical cancer was mainly HPV16.Infection with a high-risk type of HPV may more easily result in the development of cervical cancer.The timely detection of CCD4^+and CD8^+cell expression in cervical secretions is crucial to monitoring and treating HPV.
