螺旋CT与X线腹部平片在急性肠梗阻中的诊断价值

Diagnostic value of acute intestinal obstruction diagnosed with spiral CT and X-ray KUB

目的对比分析螺旋CT与X线腹部平片在急性肠梗阻中的诊断价值。方法 90例急性肠梗阻患者均为2014年5月~2016年5月期间我院收治的病例,所有患者均经过均经手术证实,90例患者均分别用X线腹部平片、螺旋CT进行诊断,经X线腹部平片进行诊断并作为对照组,经螺旋CT诊断作为研究组,比较两组患者的诊断符合率、肠梗阻类型诊断准确率、肠梗阻原因判定准确率。结果 研究组诊断符合率与对照组比较更高,误诊率与对照组比较更低,两组差异有统计学意义（P〈0.05）;研究组绞窄性梗阻、动力性梗阻诊断准确率与对照组比较更高,两组差异有统计学意义（P〈0.05）;研究组对肠粘连、肿瘤、肠麻痹的诊断准确率与对照组比较更高,差异有统计学意义（P〈0.05）。结论 X线腹部平片与螺旋CT对急性肠梗阻的诊断均有重要的临床价值,但螺旋CT在诊断准确率、分辨肠梗阻的类型及明确病因均明显优于X线腹部平片,可在临床推广应用。

Objective To compare the diagnostic value of acute intestinal obstruction diagnosed with spiral CT and X- ray KUB. Methods A total of 90 cases of acute intestinal obstruction patients were selected from May 2014 to May 2016 in our hospital, they were confirmed with surgery, all the patients were diagnosed with X-ray KUB, spiral CT, respectively, patients diagnosed with X-ray KUB and considered them as the control group, patients diagnosed with spiral CT and considered them as the study group, the compliance rates, the type of diagnostic accuracy rate of intesti- nal obstruction, intestinal obstruction reason determination accuracy rate of two groups were compared. Results The di- agnostic accuracy rate of the study group was significant higher than the control group, misdiagnosis rate was relatively lower than the control group, with significant difference between two groups（P〈0.05）; The diagnostic accuracy rate of Strangulation obstruction, dynamic obstruction in the study group was significant higher than the control group, with significant difference between two groups（P〈0.05）; The accuracy rate of adhesions, tumor, intestinal paralysis in the study group was significant higher than the control group, with significant difference between two groups（P〈0.05）. Conclusion X-ray KUB and spiral CT has important clinical value of acute intestinal obstruction, but spiral CT in diagnostic accuracy to distinguish the type of obstruction and a clear cause are significantly better than the X-ray KUB, it can be popularized in clinical application.
