摘要
目的探讨血清胱抑素C(Cys C)、同型半胱氨酸(Hcy)联合检测在2型糖尿病肾病早期诊断中的应用价值。方法按照尿清蛋白排泄率(UAER)将患者分为单纯糖尿病组(DM组,n=47),糖尿病早期肾病组(DN组,n=67)。另选同期进行健康体检者60例为对照组。分别检测和比较3组血清Cys C、Hcy、尿素氮(BUN)和血清肌酐(Scr)水平及阳性率。结果 DN组患者血清Cys C、Hcy水平高于DM组和对照组,差异均有统计学意义(P<0.05);DM组血清Cys C、Hcy水平与对照组比较,差异无统计学意义(P>0.05)。DN组中Cys C和Hcy阳性率分别为64.18%和49.25%,均显著高于Scr和BUN阳性率(P<0.05)。结论血清Cys C、Hcy能够反映早期肾功能损伤,可作为2型糖尿病肾病早期诊断指标。
Objective To explore the early diagnostic value of serum cystatin C (Cys C) and homocysteine(Hcy) in patients with type 2 diabetic nephropathy (DN). Methods A total of 114 patients with type 2 diabetic mellitus in Xiangcheng People' s Hospital from May of 2014 to April of 2015 were selected. According to urinary albumin excretion rate (UAER) , all patients were divided into simple diabetic mellitus group ( DM group, n = 47 ) and early diabetic nephropathy group ( DN group, n = 67 ). 60 healthy subjects of the same period determined by physical examination were enrolled as control groups. Serum Cys C, Hey,blood urea nitrogen (BUN) and serum creatinine (Scr) were measured and compared. Results The serum levels of Cys C, Hcy in DM group was higher than those in DM group and control group, and there were statistical difference (P 〈 0.05 ). There was no significant difference in serum levels of Cys C, Hcy between DM group and control group (P 〉 0. 05 ). The positive rate of Cys C, Hcy in DN group were 64.18% and 49.25%, significant higher than Scr and BUN. Conclusion The serum Cys C and Hcy can reflect early renal injury, and can be used as early diagnosis in type 2 diabetic nephropathy.
