黔南地区布依族与苗族女性三阴乳腺癌患者临床特征和预后影响因素比较研究

Clinical features and influencing factors of prognosis of Buyi and Miao female patients with triple-negative breast cancer in Qiannan region: a comparative study

目的探讨黔南地区布依族和苗族女性三阴乳腺癌患者临床特征和预后影响因素,为黔南地区女性乳腺癌的防治提供参考研究。方法 2005年1月-2010年6月在黔南布依族苗族自治州人民医院诊断为三阴乳腺癌的女性患者88例。按民族将其分为布依族组43例,苗族组45例,分别比较两民族临床特征和预后影响因素。结果苗族组年龄、乳腺癌家族史、肿瘤大小、组织学分级和淋巴结转移与布依族组比较,差异有统计学意义(P<0.01);苗族组复发病例、转移病例、复发时间与布依族组比较,差异有统计学意义(P<0.05)。苗族组5年无病生存率和总生存率分别与布依族组比较,差异有统计学意义(P<0.05);单因素分析结果显示乳腺癌家族史、手术方式、淋巴结转移、肿瘤大小、组织学分期、化疗周期对5年生存状况有统计学意义(P<0.05)。淋巴结转移、肿瘤大小、组织学分级、化疗周期对5年生存率的影响有统计学意义(P<0.05)。结论苗族和布依族三阴乳腺癌患者临床特征和预后影响因素有差异。临床诊疗中应高度重视。

Objective To explore the clinical characteristics and influencing factors of prognosis of triple-negative breast cancer patients of Buyi and Miao nationalities in Qiannan region, provide reference study for prevention and treatment of triple-negative breast cancer patients in Qiannan region. Methods Eighty-eight female patients diagnosed as triple-negative breast cancer in the hospital from January 2005 to June 2010 were selected and divided into Buyi naitonality group （43 female patients ） and Miao nationality group （45 female pa- tients） . The clinical characteristics and influencing factors of prognosis in Buyi naitonality group and Miao nationality group were compared. Results There were statistically significant differences in maternal age, family history of breast cancer, tumor size, histological grade, and lymph node metastasis between Buyi nationality group and Miao nationality group （ P〈0.01 ） . There were statistically significant differences in the number of recurrent cases, the number of metastasis cases, and recurrence time between Buyi nationality group and Miao nationality group （P〈0. 05 ） . There were statistically significant differences in 5-year disease-flee survival rate and the overall survival rate between Buyi nationality group and Miao nationality group （ P〈0. 05 ） . Univariate analysis showed that the influencing factors of 5-year survival situ- ation included family history of breast cancer, operation pattern, lymph node metastasis, tumor size, histological grade, and chemotherapy cycle （ P〈0. 05 ） ; the influencing factors of 5-year survival rate included lymph node metastasis, tumor size, histological grade, and chemotherapy cycle （P〈0. 05） . Conclusion There are differences in clinical features and influencing factors of prognosis of triple-negative breast cancer patients of Buyi and Miao nationalities, more attention should be paid to during clinical diagnosis and treatment.
