摘要
目的分析胰岛β细胞功能及胰岛素抵抗监测在伴有高泌乳素血症的多囊卵巢综合征(PCOS)患者中的临床意义。方法选取2013年6月-2015年6月于医院妇科门诊收治的伴有高泌乳素血症的PCOS患者400例作为观察组,另选取本院同期收治的单纯PCOS患者400例作为对照组。对两组患者的一般资料、胰岛β细胞功能指标及胰岛素抵抗指标进行比较分析。结果在一般资料方面,观察组患者痤疮发生率较对照组明显升高,差异有统计学意义(P<0.05),其他指标比较差异无统计学意义(P>0.05);在血清性激素指标方面,观察组患者的泌乳素(PRL)、睾酮(T)水平较对照组明显提高,差异有统计学意义(P<0.05);在胰岛素β细胞功能指标方面,观察组患者的胰岛素抵抗指数(HOMA-IR)较对照组明显升高,差异有统计学意义(P<0.05)。结论与单纯PCOS患者相比,伴有高泌乳素血症的PCOS患者的内分泌紊乱程度更高,胰岛素抵抗更明显。通过监测胰岛β细胞功能及胰岛素抵抗情况有助于疾病的早期诊断,对预防远期并发症具有重要的意义。
Abstract: Objective To analyze the clinical significance of islet beta cell function and insulin resistance survillence in polycystic ovary syndrome (PCOS) patients with hyperprolactinemia. Methods Four hundred PCOS patients with hyperprolactinemia were selected from Outpatient Department of Gynecology in the hospital from June 2013 to June 2015 as observation group; four hundred PCOS patients treated in the hospital during the same period were selected as control group. The general data, islet beta cell function indexes, and insulin resist- ance indexes in the two groups were compared and analyzed. Results In terms of general information, the incidence rate of ache in observation group was statistically significantly higher than that in control group ( P〈0. 05 ), but there was no statistically significant difference in other indexes between the two groups (P〉0. 05). In terms of serum sex hormones, the levels of prolactin (PRL) and testosterone (T) in observation group were statistically significantly higher than those in control group ( P〈0. 05 ) . In terms of insulin beta cell function indicators, HOMA-IR in observation group was statistically significantly higher than that in control group (P〈0. 05) . Conclusion Compared with PCOS patients, the patients with PCOS combined with hyperprolactinemia have greater degree of endocrine disorder and more significant insulin resistance. Monitoring islet beta cell function and insulin resistance are helpful in diagnosing the disease early and preventing longterm complications.
