高频超声与神经电生理检查对腕管综合征诊断价值的对比分析

目的 对比分析高频超声与神经电生理2种检测方法对腕管综合征的诊断价值。方法 选取40例（50只腕）腕管综合征患者（其中18只腕经手术治疗）分别进行高频超声和神经电生理检查,按病程长短分组,统计在病程不同阶段2种检查的阳性率,手术患者则与手术结果相对照,通过各检测指标观察和对比2种检测手段的优越性和局限性。结果 病程0.6-1年的患者中,电生理检测的异常率明显高于超声检查,差异有统计学意义（P〈0.05）;手术患者术前超声发现腕横韧带增厚（9例）、腕管内腱鞘炎（4例）、腱鞘囊肿（3例）、正中神经有瘤样改变（2例）,手术证实与超声及电生理检测相符。结论 高频超声与神经电生理检查2种检测方法相互弥补各自不足却不可互相替代,可从不同角度对CTS作出诊断和评估,为临床诊治提供重要而有价值的诊断依据。

Objective To compare the diagnostic value of high-frequency ultrasonography and neuro-electrophysiology 2 for carpal tunnel syndrome. Methods Selecting 40 cases （50 wrists） of patients with carpal tunnel syndrome （among which 18 cases have had wrist surgery）, and these patients were respectively given high-frequency ultrasound and nerve electrophysiological examination, and they were grouped according to duration of disease, the positive rates of the two examination methods in different stages were recorded; the surgical cases are in contrast with the results of surgery. The advantages and limitations of two detection means were observed through all kinds of detection of indicators and comparative analysis. Results Among patients in the course of the disease between 0.6-1 years, abnormal electrophysiological testing rate was significantly higher than ultrasound check, the difference was statistically significant （P〈0.05）; before surgery, surgical cases found transverse carpal ligament thickening （9 cases. ） through ultrasound, carpal tunnel tendon sheath （4 cases）, ganglion cyst （3 cases）, tumor-like changes in the median nerve（2 cases）, the surgery conforms to ultrasound and electrophysiological testing. Conclusion The two examination methods, high frequency ultrasound and nerve electrophysiological examination, complement each other, make up the deficiency of each other, but can not replace each other, they can make diagnosis and assessment of the CTS from different perspectives, providing important and valuable diagnostic basis for clinical diagnosis and treatment.
