高压氧联合丹参多酚酸盐治疗糖尿病周围神经病变临床效果观察

Curative Effect of Hyperbaric Oxygen Therapy Combined with Salvianolate in Treatment of Diabetic Perineuropathy

目的观察高压氧联合丹参多酚酸盐治疗糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的临床效果。方法将140例2型DPN采用随机数字表法随机分为高压氧治疗组和常规治疗组两组,各70例。高压氧治疗组在采用丹参多酚酸盐等常规治疗基础上加用高压氧治疗,常规治疗组采用丹参多酚酸盐等常规治疗。分析比较两组治疗前及治疗7、14 d后各神经症状评分、神经症状总评分(total symptom score,TSS)和密歇根神经病变筛选法(michigan neuropathy screening instrument,MNSI)评分,并观察高压氧治疗组不良反应发生情况。结果治疗前,两组各神经症状评分、神经TSS及MNSI评分比较差异均无统计学意义(P>0.05)。治疗7、14 d后,高压氧治疗组各神经症状评分、神经TSS及MNSI评分均较治疗前降低,差异均有统计学意义(P<0.01);高压氧治疗组各神经症状评分、神经TSS及MNSI评分均低于常规治疗组,差异均有统计学意义(P<0.01)。治疗14 d后,高压氧治疗组各神经症状评分、神经TSS及MNSI评分均低于治疗7 d后,差异亦均有统计学意义(P<0.05)。高压氧治疗组在行高压氧治疗过程中5例诉耳痛,未予处理及停止治疗,在治疗过程中耳痛自行缓解。结论高压氧联合丹参多酚酸盐治疗DPN效果良好,且不良反应少。

Objective To observe curative effect of hyperbaric oxygen therapy combined with Salvianolate in treatment of diabetic perineuropathy（ DPN）. Methods A total of 140 patients with type 2 DPN were divided into hyperbaric oxygen group（ n = 70） and conventional group（ n = 70） by random digits table method. Conventional group received conventional treatments such as Salvianolate,while hyperbaric oxygen group was added with hyperbaric oxygen therapy on the basis of conventional treatments. In two groups,scores of neurological symptoms,neurological total symptom score（ TSS） and Michigan neuropathy screening instrument（ MNSI） before and 7 and 14 d after treatment were analyzed and compared,and incidence rates of adverse reaction were also observed at the same time. Results Before treatment,the differences in scores of neurological symptom,neurological TSS and MNSI were not statistically significant（ P〉0. 05）. In hyperbaric oxygen group,scores of neurological symptom,neurological TSS and MNSI 7 and 14 d after treatment were significantly decreased than those before treatment（ P〈0. 01）; scores of neurological symptom,neurological TSS and MNSI in hyperbaric oxygen group 7 and 14 d after treatment were significantly decreased than those in conventional group（ P〈0. 01）. In hyperbaric oxygen group,scores of neurological symptom,neurological TSS and MNSI 14 d after treatment were significantly decreased than those 7 d after treatment（ P〈0. 05）. During the hyperbaric oxygen treatment,5 patients complained earache,and then earache was self-relieved during the treatment process without any special therapy or stopping therapy. Conclusion Hyperbaric oxygen therapy combined with Salvianolate has well curative effect in treatment of diabetic perineuropathy with less adverse reactions.
