超声乳化、人工晶状体植入联合小梁切除术治疗ACG合并Cat的临床分析

Analysis of phacoemulsification,intraocular lens implantation,and trabeculectomy in the treatment of angle-closure glaucoma merged with cataract

目的：观察超声乳化、人工晶状体植入术联合小梁切除术对闭角型青光眼（ACG）合并白内障（Cat）治疗的效果。方法：回顾性分析2015年5月-2016年5月收治的80例ACG合并Cat患者的临床资料,随机分为观察组与对照组各40例,对照组给予超声乳化、人工晶状体植入术,观察组在对照组基础上联合小梁切除术;于术前及术后1周、1个月、3个月测定两组视力、眼压、中央房前深度、视野。结果：对照组于术后1个月,观察组于术后1周开始,视力逐渐改善、眼压逐渐降低、中央房前深度逐渐增大,平均视野缺损值、平均模式标准差值逐渐改善,与术前比较差异显著（P〈0.05）,且观察组优于对照组（P〈0.05）。结论：超声乳化、人工晶状体植入术联合小梁切除术能显著改善ACG合并Cat患者视力及临床症状,值得推广应用。

Objective：To explore the clinical effect of phacoemulsification,intraocular lens implantation,and trabeculectomy in the treatment of angle-closure glaucoma merged with cataract.Methods：A total of 80 patients with angle-closure glaucoma merged with cataract who were admitted in our hospital were included in the study and randomized into the observation group and the control group（n=40）.The patients in the control group were given phacoemulsification and intraocular lens implantation.On this basis,the patients in the observation group were given trabeculectomy.The vision,intraocular pressure,central anterior chamber depth,and visual field before operation,1week,1month,and 3months after operation in the two groups were detected.Results：In the control group,1month after undergoing surgery,while in the observation group,1week after operation,patients＇ vision were significantly improved,the intraocular pressure were significantly reduced,the central anterior chamber depth was significantly increased,and the average visual field defect value and average standard deviation value were significantly improved when compared with that before undergoing operation（P〈0.05）;moreover,the improved degree in the observation group was significantly superior to that in the control group（P〈 0.05） . Conclusions： Phacoemulsification ,intraocular lens implantation,and trabeculectomy in the treatment of angle-closure glaucoma merged with cataract can effectively increase the central anterior chamber depth,reduce the intraocular pressure,and improve the vision;therefore,it deserves to be widely recommended in the clinic.
