丙泊酚复合芬太尼或地佐辛用于结肠镜下息肉摘除术的镇痛效果及神经体液变化的研究

Analgesic effects of propofol combined with fentanyl or dezocine for polypectomy under colonoscope as well as the neurohumoral changes

目的：分析丙泊酚复合芬太尼或地佐辛用于结肠镜下息肉摘除术的镇痛效果及神经体液变化。方法：选择2013年2月-2016年2月在本院接受结肠镜下息肉摘除术治疗的患者116例作为研究对象,随机分为观察组（n=58）及对照组（n=58）,在结肠镜下行息肉摘除术后,对照组患者接受芬太尼镇痛、观察组患者接受地佐辛镇痛。对比两组患者的术后6h血清吲哚类神经递质、疼痛相关指标、炎症因子、应激激素含量。结果：术后6h时,观察组患者血清中5-HTP、5-HIAA、5-HT、SP、PGE2、NGF、NPY、IL-1β、IL-6、IL-8、TNF-α、PCT、HSP70、Cor、DA、ALD、NE、ACTH的含量显著低于对照组患者,差异具有统计学意义（P〈0.05）。结论：丙泊酚复合地佐辛可以更为有效地抑制结肠镜下息肉摘除术患者的术后疼痛,同时稳定神经体液平衡。

Objective：To analyze the analgesic effects of propofol combined with fentanyl or dezocine for polypectomy under colonoscope as well as the neurohumoral changes.Methods：A total of 116 patients who received polypectomy under colonoscope in our hospital were selected as the research subjects and randomly divided into observation group and control group,58 in each group.After polypectomy under colonoscope,control group received fentanyl analgesia and observation group received dezocine analgesia.The levels of indole neurotransmitters,pain-related indexes,inflammatory factors and stress hormones in serum were compared between the two groups.Results：6hafter operation,5-HTP,5-HIAA,5-HT,SP,PGE2,NGF,NPY,IL-1β,IL-6,IL-8,TNF-α,PCT,HSP70,Cor,DA,ALD,NE and ACTH content in serum of observation group were significantly lower than those of control group.Conclusion：Propofol combined with dezocine can more effectively inhibit the postoperative pain in patients with polypectomy under colonoscope,and also stabilize neurohumoral balance.
