神经梅毒患者健康相关生命质量及其影响因素分析

Study on the Health Related Quality of Life and Its Influencing Factors in Patients with Neurosyphilis

目的分析神经梅毒患者一般状况、心理状况以及健康相关生命质量(HRQOL)评分,并统计健康相关生命质量的影响因素。方法分析一般状况,使用抑郁自评量表(SDS)评估神经梅毒患者心理状况,使用生活质量评价量表(SF-36)测量神经梅毒患者的HRQOL评分,同时使用单因素方差分析及多元逐步线性回归统计对HRQOL的影响因素进行分析。结果 SDS评分结果显示轻度抑郁52例(28.11%),中度抑郁47例(25.40%),重度抑郁13例(7.00%)。影响HRQOL评分的5个最主要因素依次为:梅毒分期、持续接受治疗、病程、锻炼、家庭和睦,偏回归系数分别为-26.17,17.64,-15.16,13.04和12.51。结论神经梅毒患者部分伴发抑郁症状,健康相关生命质量较差。在对神经梅毒的防治中,医务人员应注意心理治疗,重视梅毒分期、持续接受治疗、病程、锻炼、家庭和睦等HRQOL因素。

Objective To analysis the general condition, psychological status and health related quality of life （HRQOL） in patients with neurosyphilis, and to study the influencing factors of health related quality of life. Methods The Self-Rating depression scale （SDS） was used to assess psychological status in patients with neurosyphilis, and the SF-36 scale was used to assess the HRQOL of patients with neurosyphilis. One-way ANOVA and stepwise regression analysis were used to determine the influence factors of HRQOL. Results SDS scores showed that 28. 11% （52 casses） of patients had mild depression; Moderate depression accounted for 25.40% （47 casses） ; 13 cases （7. 00% ） were severe depression. Stepwise regression analysis showed that the major factors influencing HRQOL included the stage of syphilis, sustained treatment, course of syphilis, physical exercise and harmonious family relations with the unstandardized partial coefficients of - 26. 17, 17.64, - 15. 16, 13.04 and 12. 51, respectively. Conclusion Certain pa- tients with neurosyphilis suffer from depressive symptoms and poor HRQOL, suggesting that healthcare providers should pay attention to the factors associated with HRQOL, such as psychological therapy, stage of syphilis, sustained treatment, course of syphilis, physical exercise and harmonious family relations.
