摘要
目的超声对青年肥胖型高血压患者肱动脉内径改变和颈动脉内中膜厚度变化的探讨。方法应用高分辨率彩色多普勒超声诊断仪对40例青年肥胖型高血压组、40例正常对照组测量颈动脉内中膜厚度(Intima-Media Tickness,IMT)及肱动脉内径改变。结果青年肥胖型高血压组与正常对照组比较,各部位IMT增厚均高于对照组,其差异有统计学意义(P<0.05)。青年肥胖型高血压组肱动脉基础内径高于对照组,差异有统计学意义(P<0.05);青年肥胖型高血压组肱动脉血流介导的舒张功能(Flaw-Mediated Dilation,FMD)低于对照组(P<0.05)。青年肥胖型高血压组与对照组的动脉硬化斑块检出率比较,其差异无统计学意义(P>0.05)。高血压和肥胖两者同时存在时,FMD减低与IMT增厚具有明显负相关性(P<0.01)。结论青年人肥胖和原发性高血压与颈动脉IMT及肱动脉FMD关系密切,肥胖因素及血压因素在青年IMT及FMD中具有明显相关性。
Objective To discuss the application of ultrasound for identification of changes in brachial arteryFMD (Flow-Mediated Dilation) and carotid IMT (Intima-Media Thickness) in young patients with hypertensionand obesity. Methods High-resolution ultrasound was applied to measure the IMT and inner diameter changesof the brachial artery of 40 youth obesity & hypertension patients (Youth Obesity & Hypertension Group) and40 healthy people (Control Group). Results The IMT thickening of Youth Obesity & Hypertension Group washigher than that of Control Group, with statistically significant difference (P〈0.05). Youth Obesity & HypertensionGroup had greater baseline diameter of brachial artery and lower Flow-Mediated Dilation than Control Group,both with statistically significant difference (P〈0.05). No statistically significant difference was seen between theatherosclerosis plaque detection rates of two groups (P〉0.05). Reduced FMD and IMT thickening had obviousnegative correlation in patients with both obesity and hypertension (P〈0.01). Conclusion Essential hypertensionand obesity among young people were closely related to the brachial artery FMD and the thickness of the carotidartery IMT, indicating that obesity and blood pressure factors in youth IMT and FMD had significant correlation.
