摘要
目的比较不同浓度对比剂对64排螺旋CT冠脉成像(Multiple-Slice CT Angiography,MSCT CTA)的效果,讨论降低对比剂碘浓度的可行性。方法 200 例患者随机分成A、B两组,各100例,分别使用对比剂Visipaque(320 mg I/m L)和Ultravist(370 mg I/m L)行64排MSCT CTA,然后分析两组患者的图像质量、各测量部位的CT值、噪声值、信噪比(SNR)和对比噪声比(CNR),以及过敏反应发生率。结果 Visipaque(320 mg I/m L)组与Ultravist(370 mg I/m L)组成像质量构成之间的差异无统计学差异(P>0.05),Visipaque组与Ultravist组患者的升主动脉根部、左主干和右冠状动脉起始部的CT值、SD值、SNR和CNR之间的差异均无统计学意义(P>0.05);Visipaque组的总过敏反应发生率低于Ultravist组,差异具有统计学意义(P<0.05)。结论 Visipaque和Ultravist在冠脉成像过程中的效果基本相同,且使用Visipaque降低了病人的碘摄取量及对比剂不良反应,对于患者更适宜。
Objective To evaluate the effect of application of different iodine concentration of contrastmediums in coronary angiography with 64-slice spiral computed tomography and to discuss thefeasibility of decreasing the iodine concentration of contrast mediums. Methods Altogether 200 patientswere randomly divided into 2 groups with 100 patients in each group. Visipaque (320 mgI/mL) andUltravist (370 mgI/mL) were used in two groups respectively to perform coronary CTA (ComputedTomography Angiography) with 64-slice spiral CT. Then, the image quality, CT value, noise value, SNR(Signal-to-Noise Ratio) and CNR (Contrast-to-Noise Ratio) of each measurement site as well as the incidenceof the allergic reaction of the two groups were analyzed. Results There was no significant difference betweenVisipaque Group and Ultravist Group in the image quality (P〉0.05); there were no significant difference inthe CT value, SD value, CNR and SNR in the ascending aortic root, the left main stem and the right coronaryartery between Visipaque Group and Ultravist Group; the total allergic reaction rate of Visipaque Groupwas lower than that of Ultravist Group, with statistically significant difference (P〈0.05). Conclusion Theeffect of Visipaque and Ultravist in coronary artery imaging was basically the same, however, Visipaquewas more suitable for patients for its reduction of the patient’s iodine intake and adverse reactions.
