家庭医生责任制在社区高血压管理中的应用体会

Application of family doctor responsibility system in the management of hypertension in the community

目的 :在实施社区高血压管理过程中分析家庭医生责任制的临床应用价值。方法 :选择2014年2月—2016年0月100例社区高血压患者作为研究对象,根据干预方法不同,分为干预组50例,由"家庭医生团队"进行高血压管理;对照组50例,进行常规高血压管理,两组均干预12个月。分析两组干预前后的饮食控制、规律用药、高血压知识的掌握、运动情况、血压等指标。结果 :在高血压患者规律用药、饮食控制以及血压监测方面,干预组明显优于对照组(P<0.05);干预组收缩压和舒张压均明显低于对照组(P<0.05)。结论 :有效应用家庭医生责任制可以显著提高高血压患者对于疾病知识的知晓率,显著提高高血压治疗效果,凸显家庭医生责任制的应用价值。

Objective： In the process of implementing community hypertension management to analyze the clinical application value of family doctor responsibility system. Methods： From Feb. 2014 to Feb. 2015, 100 cases of hypertension were selected as the research objects and according to the different methods of intervention, 50 cases were an intervention group, whose hypertension was managed by family doctor team, and 50 cases were a control group, whose hypertension was managed with routine method. Both groups were managed for 12 months. The diet control, regular medication, knowledge of hypertension, exercise, blood pressure and other indicators were analyzed in the two groups before and after intervention. Results： The intervention group was significantly better than that of the control group in the aspects of regular medication, diet control and blood pressure monitoring in patients with hypertension（P〈0.05）. The levels of systolic blood pressure and diastolic blood pressure were significantly lower in the intervention group than those in the control group（P〈0.05）. Conclusion： Effective application of family doctor responsibility system can significantly improve the awareness rate of knowledge of the disease in patients with hypertension, and significantly improve the treatment effect of hypertension, which highlights the application value of the family doctor system.
