PDCA城市社区健康管理模式是将社区健康管理的4个环节与PDCA循环质量管理的4个过程相结合,从7个角度进行健康风险评估,9个方面进行健康干预,10个模块进行健康监测,并建立深入社区和家庭的健康信息获取手段、联系社区医生和居民的纽带,持续改善社区健康管理质量,解决一项健康问题后进入下一项健康管理循环,周而复始的对社区居民进行健康管理,这样阶梯式上升良性循环,以此全面保障居民健康。

PDCA urban community health management model combined the four aspects of community health management and the four process of PDCA cycle quality management , and conducted assessment health risk of seven perspective, health inter- vention of night aspects, and health monitor of ten modules. Additionally, health information acquisition means was established by the community and family, with links of community doctors and residents, and community health management quality was improved solve a health problem after a health management into the next cycle, so as to step up a virtuous cycle, and to fully protect residents health.
