县级疾病预防控制中心公共职能落实程度的10年变迁

Implementation Evaluation of Public Functions of Centers for Disease Prevention and Control at County Level from 2002 to 2012 in China

目的:分析我国县级疾病预防控制中心2002-2012公共职能落实程度变化情况。方法:采用分层随机抽样法,在全国调研257个县级疾病预防控制中心样本,调查2008和2012年样本机构公共职能项目开展比例以及开展项目的操作程度,2002和2005年落实程度结果来源于前期研究。结果:(1)从2002到2012年,县级样本疾控中心的公共职能落实程度从46.7%上升到79.0%,东中西样本机构公共职能落实程度依次递减。(2)七项职能中落实程度相对最低的是健康危害因素监测与控制,从2002-2012年七项公共职能落实程度普遍较大程度得到提高,但突发公共卫生事件应急处置、疾病预防与控制职能落实程度出现过回落情况。结论:我国县级样本疾病预防控制中心建设得到加强,各项公共职能落实程度普遍提高,未来要避免大的波动,并且重点加强健康危害因素监测与控制工作。

The Chinese Health Service Management Abstract To evaluate the change of the fulfilling degree of public functions （FDPF） of the Center for Disease prevention and controls （CDC） at county level from 2002 to 2012. Methods 257 CDCs at county level had been investigated by two-stage strat- ified sampling. The FDPF of CDCs in 2008 and 2012 was measured by multiplying average operational proportion and average oper- ational degree. And FDPF in 2002 and 2005 was taken from previous study in 2006. Results （ 1 ） The FDPF of CDCs at county lev- el increased from 46.7% to 79.0% from 2002 to 2012. And the FDPF decreased from east,middle to west of China,but the gap between the East and the Midwest of China was shrinking. （2） Among seven public functions, the fulfilling degree of monitoring and contrd of health risk factors was the lowest ; The fulfilling degree of seven public functions were improved from 8.9 to 53.5 percentage point from 2002 to 2012, respectively, but the fulfilling degree of responding to public health emergencies and disease prevention and control decreased some time. Conclusions The construction of CDCs at county level in China had been strengthened and the FDPF gained universal improvement, but they deserved continuous attention, especially the monitoring and control of health risk factors in the future.
