脑电双频谱指数指导下右美托咪定在心脏瓣膜手术中的应用 被引量:7

摘要 目的评价脑电双频谱指数(BIS)监测指导下右美托咪定在体外循环下心脏瓣膜置换手术中的应用价值。方法50例体外循环下行换瓣手术的风湿性心脏病患者随机均等分为非BIS组(N组)和BIS组(B组)。两组患者均于诱导插管前15min以右美托咪定0.5μg(kg·h)恒速泵注至术毕,B组维持术中BIS值在45~50,N组根据临床经验处理。检测静脉注射右美托咪定前(T0),体外循环建立前(T1),体外循环结束后10min(12),手术结束时(T3),术后12h(T4)和术后24h(T5)的血压、心率和动脉血皮质醇、血糖、乳酸含量;统计镇静药和血管活性药的使用量和ICU内初醒时间、拔管时间。结果与N组相比,B组患者在T2~T5时点血压和心率波动明显减小(P〈0.05);B组患者在他~T5时点皮质醇、血糖、乳酸含量明显降低(P〈0.05),丙泊酚、咪达唑仑和血管活性药明显减少(P〈0.05),初醒时间和拔管时间明显提前(P〈0.05)。结论BIS指导下应用右美托咪定可以减少体外循环下心脏瓣膜置换手术围术期的循环波动和应激反应,同时节约镇静药物和血管活性药物并促进苏醒,实现早期拔管。
