补阳还五汤配合介入法治疗糖尿病足的临床研究

The Clinical Research of Buyanghuanwu Decoction Combined with Interventional Therapy on Treatment of Diabetic Foot

目的:观察益气活血法代表方补阳还五汤配合介入方法治疗糖尿病足合并下肢动脉闭塞硬化症的临床疗效。方法:选择2014年1月至2015年12月在本院治疗糖尿病足患者60例(男54例,女6例),按随机数字法分成观察组和对照组(各30例)。给予系统、规范的基础治疗,行创面清创,对照组施行介入治疗后常规治疗,观察组在对照组的基础上服用补阳还五汤。结果:临床症状改善情况包括下肢皮温改善情况和跛行距离、静息痛、足坏疽等愈合情况,观察组比对照组较术前改善更为明显,两组伤口愈合计分,随着治疗时间的延长均呈下降趋势,但观察组伤口愈合计分下降较对照组明显(P<0.01)。结论:糖尿病足合并下肢动脉闭塞硬化症患者,在介入手段治疗下,可以使闭塞的大血管再通,联合补阳还五汤改善血供更加明显,可以缩短溃疡愈合时间,为糖尿病足提供良好的血运条件。

Objective： To observe the clinical curative effect of Buyanghuanwu decoction combined with interventional ther- apy for diabetic foot with arterial occlusive disease of lower extremity. Methods： A total of 60 patients with diabetic foot ulcers treated from January 2014 to December 2015 were randomly divided into experimental group and control group, 30 cases in each group. Each group was given the basic wound debridement, and the control group received routine treatment after interventional therapy, while the experimental group was treated with Buyanghuanwu decoction additionally. Re- suits.. The clinical symptoms including lower limb claudication distance, improve skin temperature, rest pain and gangrene were compared. Compared with the control group, the experimental group was significantly improved than the control group, the wound healing score of the two groups showed a decreasing trend with the prolongation of treatment time, but the total score of the experimental group was significantly lower than that of the control group （P〈0.01）. Conclusions： The interventional treatment can make trecanalization of blood vessel in diabetic foot combined with arterial occlusion of lower extremities. Combined with Buyanghuanwu decoction, the blood supply improvement is more obvious and it can shorten the healing time of ulcer and provide a good blood supply for diabetic foot.
