大螺距双源CT双能量扫描鉴别肺部肿瘤准确性的临床研究 被引量:4

摘要 目的:探讨大螺距CT双能量扫描鉴别肺部良性和恶性肿瘤的临床应用价值。方法:前瞻性的选取采用大螺距双源CT双能量增强扫描及病理结果证实的疑为肺部肿瘤的33例患者,测定双能量增强扫描动脉期增强图像中碘图病灶的CT值、overlay值及含碘值,以病理结果及预后情况作为金标准进行比较得出准确性。结果:与病理学结果和随访结果比较,采用大螺距双能量CT鉴别良性和恶性肿瘤的灵敏度、特异度、阳性率以及阴性率分别为80%,75%,90.9%和54.5%。恶性肿瘤的overlay值和含碘值显著高于良性肿瘤,良性和恶性肿瘤的CT值、轴位截面积及最大直径比较,差异无统计学意义(t=1.859,P>0.05)。结论:大螺距双源CT双能量肺部增强扫描有助于肺部良性和恶性肿瘤的鉴别。 Objective: To investigate accuracy of the high-pitch dual-source CT with dual-energy scan in detecting the benign and malignant lung tumor. Methods:33 patients who were suspected lung tumor and performed thoracic CT examination using high-pitch dual-source CT with dual-energy mode were selected and confirmed by histopathology or clinic follow-up. CT values, overlay values and iodine values were measured. The accuracy of lung tumor detection was acquired according to the results of pathohistology and follow-up. Results:Compared with the results of pathohistology and follow-up, the sensitivity, specificity and positive and negative predictive values of lung tumor detection using high-pitch dual-source CT with dual-energy mode were 80%, 75%, 90.9% and 54.5% respectively. The overlay values and iodine values of malignant lung tumor were significantly higher than benign tumor. The CT attention, axial cross-sectional area and the maximum diameter were no significant differences between them. Conclusion:It is helpful to use the high-pitch dual-source CT with dual-energy mode for lung enhanced CT scanning to detect the benign and malignant lung tumor.
