硬性高透氧角膜接触镜与框架眼镜矫正圆锥角膜的视觉质量差异分析

The differential analysis of visual quality between RGPCL and frame glasses for keratoconus correction

目的:探讨硬性高透氧角膜接触镜(RGPCL)和框架眼镜矫正圆锥角膜的视觉质量的差异。方法:选取圆锥角膜患者49例(98眼),根据矫正方式随机将其分为RGPCL组24例(48眼)和框架眼镜组25例(50眼),比较两组患者矫正后视力分布以及散光度、角膜曲率、高阶相差、慧差以及球差的差异,并使用日常生活问卷,比较两组患者视力矫正后日常活动主观评分的差异。结果:两组患者校正后的视力分布差异具有统计学意义(x^2=9.121,P<0.05);RGPCL组患者校正后的散光度(1.94±0.58)低于框架眼镜组(2.72±0.43),两组比较有差异(t=7.584,P<0.05);RGPCL组患者矫正后高阶相差为(0.56±0.03)高于框架眼镜组(0.48±0.09),RGPCL组患者矫正后慧差为(0.63±0.05)高于框架眼镜组(0.55±0.04),RGPCL组患者矫正后球差为(0.46±0.09)高于框架眼镜组(0.37±0.12),两组比较有差异(t=5.853,t=8.764,t=4.187;P<0.05)。RGPCL组患者矫正后看电脑的主观评分为(3.88±0.21)高于框架眼镜组(3.72±0.28),RGPCL组患者矫正后近距离阅读的主观评分为(3.90±0.24)高于框架眼镜组(3.79±0.26),两组比较有差异(t=3.190,t=2.174;P<0.05)。结论:RGPCL矫正圆锥角膜的视觉质量优于框架眼镜。

Objective：To analyze the differences of visual quality between RGPCL and frame glasses for keratoconus correction. Methods： 49 cases of 98 eyes fitting keratoconus patients in our hospital were selected and divided randomly into RGPCL group and frame glasses group according to the correction methods. We compare the corrected visual acuity distribution, degree of astigmatism, corneal curvature, high-order aberration, spherical aberration and coma between the two groups. Differences in daily activities of subjective score were compared between the two groups after correction by use of daily life questionnaire. Results：The differences of visual acuity distribution was statistically significant between RGPCL group and frame glasses group （x2=9.121, P〈0.05）. The astigmatism of RGPCL group （1.94±0.58） after correction was lower than the frame glasses group （2.72±0.43）, and the difference has statistical significance （t=7.584, P〈0.05）. The high order aberrations （0.56±0.03） and the spherical aberration （0.48±0.09） of RGPCL group after correctionand was higher than that （（0.46±0.09） （0.37±0.12）） of the frame glasses group. The difference has statistical significance （t=5.853, t=8.764, t=4.187;P〈0.05）. The subjective scores of look at computer in RGPCL group （3.88±0.21） after correction was higher than the frame glasses group （3.72±0.28）, the subjective score of nearly reading in RGPCL group （3.90±0.24） after correction was also higher than the frame glasses group （3.79±0.26）, and the difference has statistical significance （t=3.190, t=2.174;P〈0.05）. Conclusion：The visual quality of rigid high oxygen permeable contact lens for keratoconus is better than the rigid frame glasses.
