摘要 目的:探讨血清肿瘤标志物人附睾蛋白4(HE4)、细胞角蛋白19片段(CYFRA21-1)和组织多肽特异性抗原(TPS)联合检测在非小细胞肺癌(NSCLC)早期诊断中的应用价值。方法:应用电化学发光法和ELISA分别测定NSCLC患者、肺部良性病变患者及健康对照组人员血清中HE4、CYFRA21-1和TPS水平.比较3组间HE4、CYFRA21—1和TPS水平与阳性率的差异.以及联合检测诊断NSCLC的敏感性及特异性。结果:(1)肺癌患者整体TPS、CYFRA21-1和HE4水平及I~Ⅱ期NSCLC患者HE4水平显著高于肺部良性病变患者及健康体检者(P〈0.05);Ⅲ~Ⅳ期NSCLC患者TPS、CYFRA21-1和HE4水平高于I-ⅡNSCLC患者(P〈0.05)。(2)HE4、CYFRA21—1和TPS联合检测诊断NSCLC的敏感性较单项检测敏感性明显增高(P〈0.05)。结论:血清HE4检测比CYFRA21.1和TPS对于NSCLC有更高的早期诊断价值。血清HE4、CYFRA2I-1和TPS联合检测对于NSCLC的诊断较单一指标具有更高的诊断价值。 Objective To investigate the application of the combined detection of HE4 with CYFRA21-1 and TPS antigen in the early diagnosis of non-small cell lung cancer. Methods The electrochemilumineseence and ELISA were used to detect the levels of serum HE4, CYFRA21-1 and TPS in patients with non-small cell lung cancer, and patients with benign lung disease and the healthy controls. The differences of the serum HE4, CYFRA21-1 and TPS, and the positive rate among the three groups were compared. The sensitivity and specifici- ty of the different combined detection methods for NSCLC were analyzed. Results (1) The TPS, CYFRA21-1 and HE4 level in patients with lung cancer and the HE4 level in patients with the I-]I stage NSCLC were signif- icantly higher than those in patients with benign lung disease and in the healthy controls (P 〈 0.05, respective- ly); The TPS, CYFRA21-1 and HE4 level in patients with the m-IV NSCLC were higher than those in the pa- tients with the I~Ⅱ NSCLC (P 〈 0.05). (2)Compared with the single detection, the sensitivity of the combined detections of TPS, CYFRA21-1 and HE4 for the diagnosis of NSCLC was significantly increased (P 〈 0.05). Conclusion Higher value of early diagnosis for NSCLC was shown in detection of the serum HE4 than the CYFRA21-1 and TPS. The combined detection of Serum HE4, CYFRA21-1 and TPS for NSCLC diagnosis showed higher diagnostic value than the single detection.
