结构化教育对2型糖尿病患者干预效果的系统评价 被引量:11

Interventional Effect of Structured Education on Patients with Type 2 Diabetes Mellitus:A Systematic Review

摘要 目的系统评价结构化教育对2型糖尿病(type 2diabetes mellitus,T2DM)患者的干预效果。方法计算机检索中国生物医学文献服务系统(SinoMed)、中国学术期刊网全文数据库(China national knowledge infrastructure,CNKI)、万方数据库、中国科技期刊数据库、PubMed数据库、Web of Science引文索引数据库、Elsevier Science Direct全文期刊数据库等数据库,收集2000-2015年国内外公开发表的关于结构化教育应用于T2DM患者的随机对照试验(randomized controlled trial,RCT),并采用RevMan 5.3统计软件进行分析。结果最终纳入9篇RCT文章,共3342例T2DM患者。Meta分析结果显示:观察组患者的糖尿病(diabetes mellitus,DM)知识、自我管理行为量表(summary of diabetes self-care activities scale,SDSCA)、DM管理自我效能量表(diabetes management self-efficacy scale,DMSES)得分均明显高于对照组(均P<0.01);但在改善糖化血红蛋白(glycosylated hemoglobin,HbA1c)方面,与对照组比较,差异无统计学意义(P>0.05)。结论结构化教育能够提高T2DM患者的DM相关知识水平以及自我效能感,改善患者的自我管理行为。 Objective To systematically review the interventional effect of structured education on patients with type 2 diabetes mellitus (T2DM).Metbods Such databases as Chinese biomedicine literature database (SinoMed), China national knowledge infrastructure (CNKI), Wanfang, VIP, PubMed, Web of Science and Elsevier Science Direct were searched to collect the random controlled trials (RCTs)about intervention effect of structured educa- tion on patients with T2DM which were published in China and abroad from 2000 to 2015.RevMan5.3 was used to analyze the data. Results Nine RCTs involving 3342 participants were included. Meta analysis showed that the scores of diabetes mellitus (DM)knowledge, summary of diabetes self-care activities scale (SDSCA), DM diabetes management self-efficacy scale(DMSES) in the observation group were significantly higher than those in the control group (all P〈0.01), while in improving glycosylated hemoglobin (HA1c) ,the difference was not statistically significant (P〉0.05).Conclusions Structured education can improve DM knowledge, self-efficacy, and self-management behaviors of patients with T2DM.
