针刀松解膝内侧副韧带治疗合并内翻畸形的膝关节骨性关节炎

Using acupotomy release medial collateral ligament for the osteoarthritis of knee with genu varum

目的:探讨针刀松解膝内侧副韧带(MCL)治疗合并内翻畸形的膝关节骨性关节炎(KOA)的早期临床疗效。方法:回顾性分析采用针刀松解膝关节内侧副韧带治疗的87例合并内翻畸形的KOA患者的临床资料,对患者术前、术后膝关节疼痛及关节功能进行评估(WOMAC评分法),比较术前、术后、术后6个月WOMAC评分。结果:平均随访8个月,术后及术后6月随访时关节疼痛、僵硬及关节活动困难WOMAC评分较术前减小,差异具统计学意义;但治疗后6个月疼痛及僵硬评分较治疗后评分增高,差异具统计学意义。9例(18膝)复发,其中6例接受双膝关节表面置换术(TKA)治疗,3例因严重内科疾病不能耐受TKA,而再次接受针刀治疗。结论:对于合并内翻畸形的KOA患者,针刀松解膝内侧副韧带治疗能够改善症状、恢复膝关节部分功能,对于晚期的KOA患者能够推迟其行TKA手术时间。

Objective：To explore the early clinical curative effect and surgical technique of using acupotomy release medial collateral ligament for the osteoarthritis of knee with genu varum.Methods：Retrospective analysis the clinical data of 87 cases osteoarthritis of knee with genu varum treat by acupotomy release medical collateral ligament.Using the WOMAC score method to evaluate the knee joint pain and joint function before and after surgery.Compare the scores of before and after surgery and 6months after operation.Results：All patients were followed up for an average of 8months.The WOMAC score of knee joint pain and stiffness and joint activity was decreased,and the difference was statistically significant in 9patients（18knees）recurrence,which 6cases received double knee arthroplasty surgery（TKA）treatment,3cases because of serious diseases can not tolerate TKA which received another acupotomy therapy.Conclusion：For the patients of KOA with genu varum,the therapy of using acupotomy release medial collateral ligament can improve the symptoms and restore the function of the knee joint.For patients with advanced KOA who received the acupotomy treatment,the time of the TKA operation can be delayed.
