1086例体检人群的结肠镜检查结果分析

Study on the colonoscopy results of 1086 cases in asymptomatic population

目的 探讨结肠镜检查在体检人群中的意义.方法 对北京军区总医院消化内镜中心2014年1月至2015年7月接受结肠镜检查的1086例无症状体检人群的临床资料进行分析.结果 1086例无症状体检人群中男性645例、女性441例,平均年龄（47.8±9.7）岁,检出结肠息肉210例（19.3％）,共329枚,其中腺瘤性息肉173例（15.9％）,中-重度异型增生腺瘤性息肉33例（3.0％）、结直肠炎47例（4.3％）、直肠癌3例（0.3％）.结肠息肉检出率中男性24.7％,显著高于女性（11.6％）（P＜0.01）.中-重度异型增生腺瘤性息肉检出率随着年龄的增长而逐渐增加,41～50岁、51～60岁及≥61岁组检出率均高于40岁及以下年龄组（P＜0.05）;息肉直径多＜0.5cm;病理类型以管状腺瘤居多.结论 结肠镜检查是早期发现并诊断结直肠息肉、结直肠癌等病变的安全有效途径.无症状体检者结肠病变以结肠息肉居多,男性及年龄＞50岁者结肠息肉检出率较高.

Objective To investigate the significance of colonoscopy in health examination of asymptomatic population. Methods A retrospective analysis was carried out on clinical data of 1 086 asymptomatic persons undergoing colonoscopy in our center. Results 1 086 asymptomatic persons （645 male and 441 female） accepted colonoscopy, with an average age of 47.8 ± 9.7 years old. Among 1 086 cases, there were 210 cases （ 19.3% ） with colorectal polyps with 329 polyps including adenomatoid polyps in 173 case （ 15.9% ）, moderate-severe dysplasia polyps in 33 case（3.0% ） ,47 cases （4.3%） of colitis and 3 cases （0.3%） of rectal cancer. The detection rates of colorectal polyps was significantly higher ill males than in females （24.7% vs 11.6% ,P 〈 0.01 ）. The detection rate of moderate-severe dysplasia adenomatoid polyps increased with age. The rates in 41-50,51-60 and ≥ 61 years old age groups were higher than in ≤40 years oht age group （P 〈 0.05 ）. Most of the polyps were 〈 0.5 cm in diameter and tubular adenoma in pathological pattern. Conclusions Colonoscopy is an effective method for screening colorectal polyps and colorectal cancer. Most of the colon lesions in asymptomatic persons are colonic polyps. The detection rates of colonic polyps are higher in males and in aged 〉 50 years.
