摘要 目的:通过分别采用Er:YAG激光与高速涡轮金刚砂车针处理老年人根面龋,观察患者的术中疼痛反应、去龋时间及充填效果。方法:共20位患者52颗患牙,随机分为2组,分别用Er:YAG激光和高速涡轮金刚砂车针去龋,去龋完成后用玻璃离子充填窝洞,记录两种方法去龋所用时间及术中疼痛程度,对患牙术后1个月后有无过敏感觉、6个月后充填体边缘着色,边缘密合等情况进行分析评价。结果:Er:YAG激光组较高速涡轮金刚砂车针组术中疼痛感明显降低,(P=0.0026);Er:YAG激光组和高速涡轮金刚砂车针组去龋时间分别为(394.8±27.92)和s(132.1±8.438)s。1个月复查,两组患者均未出现自发痛,牙本质过敏症状也无明显差异(P=0.159);术后六个月,两组充填体边缘着色(P=0.638)、边缘密合性(P=0.149)及脱落情况(P=0.685),均无明显差异。结论:Er:YAG激光组在去龋过程中疼痛感明显低于高速涡轮金刚砂车针组,但Er:YAG激光去龋所用时间更长,两组充填术后的疗效观察无明显差异。 Objective:Er:YAG laser and high speed turbine were used to deal with root caries in the elderly.Observe the intraoperative pain response,of elderly patients,the time of caries removal and postoperative filling effect.Methods;A total of 20 patients with 52 teeth were selected.Then all teeth were randomly divided into 2 groups,caries were removed by Er:YAG laser and high speed turbine emery needle,respectively,After the caries removal,the cavity was filled with GIC,and recorded the time of caries removal and intraoperative pain degree by the two methods,and proceeded analysis and evaluation about the tooth allergy feeling three month after operation,the situation of obturator edge coloring and the edge seal six month after operation.Results:The time of caries removal in Er:YAG laser group and high speed turbine emery needle group is(394.8±27.92)s and(132.l±8.438)s,respectively.The time in the two groups have significant difference(P〈0.01).The pian in Er:YAG laser group is significant lower than high speed turbine emery needle group,and differences are statistically significant(P=0.0026).A month after operation and the re-examination shows:there is no spontaneous pain occurred and there is no significant difference of the symptom of hypersensitive dentin in two groups(P=0.159).Six months after operation,there is no significant difference about the obturator edge coloring(P=0.63 8),the edge seal(P=0.149)and GIC wear(P=0.685).Conclusions;The pian of Er:YAG laser group in the process of removal caries is significant lower than high speed turbine emery needle group,but the time is longer.The short-term effects of the two groups after filling have no significant difference.
