基于团队的认知干预对直肠癌造口患者自我效能、自护能力及术后恢复的影响

Effect of self efficacy,self-care ability by team based cognitive intervention for patients with rectal cancer ostomy

目的探讨基于团队的认知干预对直肠癌造口患者自我效能、自护能力及术后情况的影响。方法随机将160例直肠癌患者分为观察组和对照组,每组各80例。两组患者给予基础护理,观察组另给予团队认知干预。随访记录两组患者术后3月并发症及造口周围皮肤情况,并对其自我效能、自护能力及术后恢复情况进行比较。结果观察组术后3月并发症总计发生率为45.00%,对照组为15.00%,差异有统计学意义(P<0.05)。观察组皮肤病变情况要明显优于对照组(P<0.05)。观察组自我效能、自护知识及自护技能得分均高于对照组;出院3月后观察组各项评分仍高于对照组,且观察组组内出院3月后各项评分较出院第一天均有提高,差异均有统计学意义(P<0.05)。结论对直肠癌造口患者进行基于团队的认知干预可有效增强患者自我效能、自护能力,有利于减少术口周围皮肤病变,降低术后并发症的发生率。

Objective To investigate the effect of Self efficacy, self-care ability nd postoperative condition by Team based cognitive intervention for patients with rectal cancer ostomy. Method 160 cases with rectal cancer were randomly divided into observation group and control group. Patients in the control group were given basic nursing, and the observation group was given the team cognitive intervention. Recorded two groups＇ postoperative complications after 3 mouths and stoma skin around the situation, and the self-efficacy and self-care ability were compared with each other. Results The postoperative complications rate of observation group was 45.00% and15.00% in the control group, the difference was statistically significant（P〈0.05）. The skin lesions of the observation group was significantly better than that of the control group（P〈0.05）. Observation group＇s self-efficacy and self-care knowledge and self-care skill scores were higher than those in the control group;and after leaving hospital 3 mouths, the observation group＇s scores was still higher than the control group, and the every score of observation group after leaving hospital 3 mouths were improved, the difference was statistically significant（P〈0.05）Conclusion Effect of Self efficacy, self-care ability by Team based cognitive intervention for patients with rectal cancer ostomy is good,and it can decrease the complications.
