分析腹腔镜和开腹手术治疗异位妊娠保留生育功能的优劣差异

Difference of fertility preserving function between laparoscopic and open surgery in treatment of ectopic pregnancy

目的研究腹腔镜和开腹手术治疗异位妊娠保留生育功能的优劣差异。方法随机选取2014年1月至2016年6月进行治疗的异位妊娠并要求保留生育能力的82例患者,回顾性分析其临床资料,将进行开腹手术的40例患者作为对照组,将进行腹腔镜手术的42例患者作为观察组。随访一年,对两组的手术效果及相关指标进行对比。结果观察组在手术时间、出血量、术后肛门排气时间及住院时间等指标明显优于对照组(P<0.05);随访一年,观察组的输卵管通畅情况、再次异位妊娠及正常妊娠发生情况与对照组比较,无显著差异(P>0.05)。结论将腹腔镜手术应用于异位妊娠,能够减小创伤、降低出血量、缩短住院时间,术后康复快,值得临床推广应用。

Objective To study the difference of fertility preserving function between laparoscopic and open surgery in the treatment of ectopic pregnancy. Methods Randomly selected in January 2014 to June 2016 for treatment of ectopic pregnancy and preserve fertility 82 patients, a retrospective analysis of the clinical data, 40 patients receiving open surgery as the control group, 42 patients with laparoscopic surgery as the observation group. During one year follow-up, the two groups were compared with the results of surgery and related indicators. Results In the observation group in the operation time, bleeding volume, postoperative anal exhaust time and the length of time the index was significantly better than the control group （ P 〈 0. 05 ） ; all the patients were followed up for one year, the observation group of fallopian tube patency, ectopic pregnancy again and normal pregnancy incidence compared with the control group, no significant difference （P〉 0. 05）. Conclusions The application of laparoscopic surgery in ectopic pregnancy can reduce the trauma, reduce the amount of bleeding, shorten the hospital stay, postoperative recovery is fast, it is worthy of clinical application.
