羊膜移植联合秦皮滴眼液和α-干扰素治疗翼状胬肉疗效观察

Effect of amniotic membrane transplantation combined with Qinpi Eye Drops and α-interferon for the treatment of pterygium

目的探讨羊膜移植联合秦皮滴眼液和α-干扰素治疗翼状胬肉的疗效。方法选择广州市中西医结合医院眼科2012年1月至2014年5月收治的翼状胬肉患者100例共100只眼作为研究对象。按随机数字表法将所有患者平均分为观察组及对照组，每组50例50只眼。对照组患者接受翼状胬肉手术切除联合羊膜移植治疗，观察组患者在此基础上联合秦皮滴眼液及α-干扰素治疗。比较两组患者的治愈率和复发率，术后创面愈合时间及不良并发症发生情况。结果观察组和对照组患者治愈率分别为96.0％和76.0%，观察组显著高于对照组，差异有统计学意义（χ^2=8.306，P =0.004）；观察组和对照组患者的复发率分别为6.0％和24.0%，观察组显著低于对照组，差异有统计学意义（χ^2= 6.353，P =0.012）。观察组和对照组患者创面愈合时间分别为（6.0±1.0）d和（9.1±1.3）d，观察组显著少于对照组，差异有统计学意义（t=13.365，P 〈0.01）。观察组和对照组患者术后发生角膜结膜水肿、创面愈合不良及睑球粘连等不良并发症的发生率分别为6.0％和20.0%，观察组显著低于对照组，差异有统计学意义（χ^2=4.332，P =0.037）。结论使用手术切除后羊膜移植联合秦皮滴眼液和α-干扰素治疗翼状胬肉疗效确切，治愈率高，复发率低，并可有效降低不良反应发生率，值得临床推广应用。

Objective To evaluate the effect of amniotic membrane transplantation combined with Qinpi Eye Drops and a-interferon for the treatment of pterygium. Methods One hundred pa- tients （100 eyes） with pterygium in Guangzhou Hospital of Integrated Traditional Chinese and West- ern Medicine from January 2012 to May 2014 was selected as the research objects, all of them were divided into two groups with 50 cases （50 eyes） in each group according to the random number table method. The control group used pterygium excision combined with amniotic membrane transplantation, while the study group was treated with Qinpi Eye Drops and u-interferon on the basis of that. The curative effect, the wound healing time and the occurrence of complications of the two groups were compared. Results The cure rate of the study group and the control group were 96.0% and 76.0%, respectively. The study group was significantly higher than the control group （c2= 8.306, P =0.004）. The wound healing time of the study group was （6.0±1.0） d, which was significantly shorter than （9.1±1.3） d of the control group （P 〈0.05）. In the study group, the postoperative complications such as corneal conjunctival edema, wound undesirable healing, and symblepharon were 6.0%, significantly lower than 20.0% of the control group （P 〈0.05）. Conclusions Using amniotic membrane transplantation after surgical resection combined with Qinpi Eye Drops and α-interferon has an exact effect for the treatment of pterygium, with the low recurrence rate, and reduce the incidence of complications, which is worthy of clinical application.
