紫杉醇脂质体联合复方苦参注射液、益气养胃汤治疗晚期胃癌伴有恶性腹腔积液临床疗效评价

Observation of Paclitaxel Liposome Fufang Kushen Injection and Yiqi Yangwei Decoction Combined with in the Treatment of Advanced Gastric Cancer with Malignant Ascites

目的:探究腔内注射紫杉醇脂质体联合复方苦参注射液,配合口服益气养胃汤治疗恶性腹腔积液的临床疗效。方法:我科98例晚期胃癌伴有腹腔积液患者,随机分为3组,分别应用腹腔内注射紫杉醇脂质体(A组)、腹腔内注射紫杉醇脂质体联合复方苦参注射液(B组)、腹腔内注射紫杉醇脂质体联合复方苦参注射液及口服益气养胃汤(C组),比较3组临床疗效及副作用。结果:A组总有效率54.54%,B组总有效率78.13%,C组总有效率81.82%优于A、B组,差异具有统计学意义(P<0.05)。毒副反应方面,C组恶心呕吐症状与A、B组相比,差异具有统计学意义(P<0.05)。治疗后,C组生活质量评分优于A、B组(P<0.05)。结论:紫杉醇脂质体联合复方苦参注射液、益气养胃汤,针对晚期胃癌伴有恶性腹腔积液患者临床治疗效果显著,患者生活质量改善明显,可有效延长患者生存期,安全可靠,值得推广应用。

Objective：To explore the clinical effect of paclitaxel liposome and Fufang Kushen Injection,combined with Yiqi Yangwei Decoction in the treatment of advanced gastric cancer with malignant ascites. Methods：98 cases of advanced gastric cancer patients with intra-abdominal fluid in our department were randomized into 3 groups. Patients in group A were given articular injection of paclitaxel liposome. Patients in group B were given articular injection of paclitaxel liposome and Fufang Kushen Injection. Patients in group C were given articular injection of paclitaxel liposome and Fufang Kushen Injection,combinedwith Yiqi Yangwei Decoction. The clinical effects and side effects were compared among the three groups. Results：A total effective rate was 54.54% in group A,78.13% in group B,and 81.82% in group C. The difference was statistically significant（P〈0.05）. The incidence of nausea and vomiting in group C were inferior to that in group A and B（P〈0.05）. The patients＇ quality of life in group C was superior to that in group A and B（P〈0.05）. Conclusion：The articular injection of paclitaxel liposome and Fufang Kushen Injection,combined with Yiqi Yangwei Decoction administration can effectively reduce the amount of ascites and suppress ascites formation in the treatment of advanced gastric cancer with malignant ascites. The quality of life and survival time are significantly improved. The clinical effects are obvious and worthy of promoting widely.
