宫术宁胶棒利多卡因和阿托品用于人工流产临床效果观察

目的探讨宫术宁胶棒、利多卡因、阿托品减少人工流产中受术者痛苦的效果。方法选取2008年6月至2015年12月人工流产妇女328例。其中2008年6月至2012年5月160例未使用任何药物直接进行负压吸宫人工流产为对照组，2012年6月至2015年12月168例为观察组，受术者术前宫颈管置宫术宁胶棒（达宫颈内口），10min后取出胶棒，在宫颈旁3点、9点处各注入2％利多卡因，再在宫颈11点处注入阿托品．30s后行负压吸宫操作同对照组。记录并观察两组术中疼痛程度、宫颈松弛度、术中出血量，人工流产综合反应发生情况。结果与对照组比较，观察组疼痛程度显著降低（镇痛有效率91．67％，P〈0．05），未发生人工流产综合反应（发生率0％，P〈0．05），宫颈松软（松弛度98．81％，P〈0．05），两组出血量均较少（P〉0．05）。结论宫术宁胶棒联合利多卡因、阿托品用于人工流产镇痛效果明显，可阻止人工流产综合反应发生，特别适合基层医疗机构推广应用。

Objective To investigate the effect of pain-reducing in induced abortion by Disposable Cervical dilatation stick, lidocaine and atropine. Method Selecting 328 cases of induced abortion from June 2008 to December 2015; choosing 160 cases between June 2008 and May 2012 as the control group that didn＇t use any drug to do induced abortion and 168 cases between June 2012 and December 2015 as the observation group. Before operation, the person who accepted the operation were placed with the disposable cervical dilatation stick in cervical tube, and the glue stick was removed after 10 minutes. After that, 2% lidoeaine was injected in the point 3 and 9 next to the cervical, and atropine was injected in the point 11 in the cervical. After 30s, operation were performed the same as in the control group. Recording and observing each reaction （ degree of pain, relaxation of cervix, amount of bleeding during operation, synthetic reaction of induced abortion ） . Results Compared with the control group, the degree of pain in the observation group was significantly lower （ Effective analgesia is 91.67%, P〈0.05 ） , and synthetic reaction of induced abortion did not occur （ Incidence is 0%, P〈0.05 ） , the cervix is soft （ laxity is 98.81%, P〈0.05 ） , the amount of bleeding in the two groups were less （ P〈0.05 ） . Conclusion Disposable cervical dilatation stick combined with lidocaine and atropine for analgesia in induced abortion has significant effects, and they can prevent synthetic reaction of induced abortion, especially suitable for the popularization and application of primary medical institutions.
