不同阿奇霉素静脉滴注时间对小儿肺炎支原体肺炎治疗效果及患儿依从性的影响 被引量:34

摘要 目的探讨不同阿奇霉素静脉滴注时间对肺炎支原体肺炎患儿的临床治疗效果及依从性的影响。方法 120例支原体肺炎患儿采用随机数字表法分为A、B、C 3组各40例。3组患儿均采用10 mg/(kg·d)阿奇霉素静脉滴注,1次/d,共6 d,A组、B组、C组的静脉滴注时间分别为30 min/次、2 h/次、4 h/次。比较3组临床症状缓解时间、不良反应发生情况、临床疗效、患儿依次性、治疗前后白细胞介素(IL)-2、IL-13水平。结果 B、C组发热、咳嗽、喘息、肺部啰音缓解所需时间均短于A组(P<0.05),治疗效果好于A组(P<0.05),但B组与C组间症状缓解所需时间及治疗效果比较,差异均无统计学意义(P>0.05)。A、B组患儿治疗依从性高于C组(P<0.05);A、B两组不良反应率均低于C组(P<0.05)。结论采用阿奇霉素静脉滴注时间为2 h/次治疗小儿肺炎支原体肺炎效果最佳。 Objective To investigate the influence of azithromycin with different intravenous infusion durations on the therapeutic efficacy and compliance in children with Mycoplasma pneumoniae pneumonia. Methods A total of 120 children with Mycoplasma pneumoniae pneumonia were divided into Group A,Group B and Group C using a random number table,with 40 cases in each group. Intravenous infusion of azithromycin with the dose of 10 mg/(kg · d) was performed in the three groups for 6 days(once for one day). And the intravenous infusion durations of Group A,Group B and Group C were 30 minutes,2 hours and 4 hours for each time respectively. The remission duration for clinical symptoms ,incidence of adverse events, clinical efficacy, compliance of children, interleukin (IL)-2, IL-13 levels before and after treatment were compared among the three groups. Results The remission durations of fever,cough,wheezing,pulmonary rales in Group B and C were shorter than those in Group A(P 〈 0.05),the therapeutic efficacies of Group B and C were superior to that in Group A(P 〈 0.05). But there were no statistical differences in the remission duration for clinical symptoms or therapeutic efficacy between Group B and Group C ( P 〉 0.05 ). The compliance of children in Group A and B were higher than that in Group C (P 〈 0.05). The incidence rates of adverse reactions of Group A and B were lower than that of Group C (P 〈 0.05). Conclusion Intravenous infusion of azithromycin with 2 hours for each time obtains optimal therapeutic efficacy for children with Mycoplasma pneumoniae pneumonia.
