帕瑞昔布钠对急性脑梗死患者疗效、炎症指标及肾功能的影响

Effect of parecoxib sodium on efficacy,inflammation indicators and renal function in patients with acute cerebral infarction

目的探讨帕瑞昔布钠对急性脑梗死(ACI)患者疗效、炎症指标及肾功能的影响。方法选择ACI患者89例,随机分为观察组46例和对照组43例。两组均给予常规治疗及尿激酶溶栓治疗,观察组加用帕瑞昔布钠治疗。治疗前及治疗1周后,比较两组患者的炎症和肾功能指标水平、美国国立卫生研究院卒中量表(NIHSS)评分、急性生理与慢性健康评估(APACHEⅡ)评分。结果治疗后两组的超敏C反应蛋白、肿瘤坏死因子-α(TNF-α)、白细胞介素-6、胱抑素、血尿素氮、血清肌酐水平以及APACHEⅡ、NIHSS评分均较前降低(除外TNF-α,均P<0.05),且观察组以上指标均低于对照组(均P<0.05)。结论帕瑞昔布钠能够改善ACI患者的神经功能缺损情况及保护肾功能,其可能通过抑制炎症介质的大量释放、控制炎症反应而实现。

Objective To explore the effect of parecoxib sodium on efficacy,inflammation indicators and renal function in patients with acute cerebral infarction（ACI）. Methods Eighty-nine patients with ACI were enrolled and were randomly divided into observation group（ n =46） and control group （n = 43 ）. Routine therapy and urokinase thrombolysis were performed in both groups. Additionally, the observation group was administered parecoxib sodium. Before treatment and after 1 week of treatment, the levels of inflammation and renal indicators ,the scores of National Institute of Health Stroke Scale（NIHSS） and Acute Physiology and Chronic Health Evaluation Ⅱ（ APACHE Ⅱ） were compared between the two groups. Results After treatment,the levels of high-sensitivity C-reactive protein,tumor necrosis-α,interleukin-6, eystatin C,blood urea nitrogen and serum creatinine, and the scores of APACHE Ⅱ and NIHSS decreased in both groups compared to those before treatment（P 〈0. 05 ）,and the indices above in the observation groups were lower than those in the control group （all P 〈 0.05 ）. Conclusion Parecoxib sodium can improve the neurological function defect and protect the renal function of patients with ACI probably by inhibiting the release of inflammatory mediators and controlling the inflammatory response.
