渗湿明目疗法治疗视网膜分支静脉阻塞型黄斑水肿研究

Clinical analysis of ShenShiMingMumethod for BRVO-ME(yin deficiency and yang excess)

目的：初步探讨＂渗湿明目疗法＂治疗＂阴虚阳亢型＂视网膜分支静脉阻塞继发黄斑水肿的疗效及安全性。方法：回顾性分析2015年2月—2016年10月就诊于我院眼科的＂阴虚阳亢型＂视网膜分支静脉阻塞继发黄斑水肿患者34例（34眼）,疗程1-23个月,所有患者均给予＂渗湿明目疗法＂治疗,包括针刺日1次、直流电血栓通离子导入、天麻钩藤饮化裁方中药汤剂口服,比较治疗前及治疗后10天、1个月、2个月的最佳矫正视力（BCVA）、眼压,黄斑中心视网膜厚度（CMT）,观察治疗后效果及不良反应。结果：治疗前及治疗后10天、1个月、2个月的最佳矫正视力分别为（0.18±0.09）、（0.27±0.15）、（0.38±0.07）、（0.42±0.14）,治疗后各时间点均较治疗前明显提高,差异有统计学意义（均P〈0.05）;治疗前与治疗后眼压均在正常范围,差异均无统计学意义;治疗前及治疗后10天、1个月、2个月患眼的黄斑中心视网膜厚度分别为（515.42±213.66）μm、（498.45±179.66）μm、（381.34±109.86）μm、（271.34±79.46）μm,治疗10天后CMT水平与治疗前相比差异无统计学意义（P〉0.05）,治疗1个月、2个月后黄斑水肿明显改善,与治疗前相比差异均有统计学意义（均P〈0.05）。治疗后未见严重不良反应。结论：＂渗湿明目疗法＂可以有效提高＂阴虚阳亢型＂视网膜分支静脉阻塞继发黄斑水肿患者的视力,能够促进黄斑水肿的吸收,治疗后眼压正常,且无明显不良反应。

Objective ： Preliminary discuss on the efficacy and safety of ShenShiMingMumethodwhich used to treat BRVO - ME （ yin deficiency and yang excess）. Methods：We take a retrospective analysis about 34 cases （34 eyes） of BRVO - ME which diagnosed by our hospital in February 2015 to October 2016 ,time of treating last from 1 to 23 months. All patients were treated by the method of ＂Shen Shi Ming Mu＂ ,which including acupuncture, direct current iontophoresis of XueShuanTong injection and decoction of herbal medicine changed by TianMaGouTengYin. Compair the BCVA , IOPand CMT before treatment with BCVA , IOPand CMT of the tenth day, the first month and the second month after treatment. Results ： BCVA of patients before treatment is 0. 18 ± 0.09, BCVA of patiens of the tenth day after treatment is 0.27 ±.15, the first month after treatment is 0.38 ± 0.07, the second month of treatment is 0.42 ±0. 14. BCVA of patients is developed after treatment at all point in time,the difference is significant in aspect of statistical,P 〈 0. 05 ;IOP of patients is in normal range before therapy and when the treatment end it is also in normal range, the difference is pointless in aspect of statistical . CMT of patients before treatment is （ 515.42 ±213.66） μm , CMT of the patients of the tenth day after treatment is （498.45 ± 179.66）μm,CMT of patients of the first month after treatment is （381.34 ±109.86 ） μm, CMT of patients of the second month after treatment is （271.34 ± 79.46 ）μm. When patients treat about 10 days, compairing with CMT before treatment, the difference is pointless in aspect of statistical, P 〉 0.05. It shows good effects on reducing CMT when treatment last for 1 and 2 month. Compairing with CMT before treatment, the difference is significant in aspect of statistical, P 〈 0. 05. There is no Adverse reactions during treatment. Conclusion： henShiMingMu method can show good effect on developing BCVA of BRVO - ME patients （ yin deficiency and yang excess） , promoting the absorption of ME. IOP is in normal range after treatment and no Adverse reactions .
