颈后肌群劳损对颈椎病患者颈曲改变的影响及针刀治疗研究

Cervical Flexure Change of Patients with Cervical Spondylosis Suffering Neck Muscle Strain and Effect of Acupotomy Treatment

目的:探讨颈后肌群劳损对颈椎病患者颈曲改变的影响及针刀治疗策略,为临床针刀治疗颈曲改变提供思路。方法:通过颈后肌群解剖学特点及肌动学特征,探讨颈后肌群劳损对颈曲改变的影响;临床将120例颈曲改变的颈椎病患者随机分为颈部针刀治疗组、肩背部针刀治疗组和颈肩背部针刀治疗组,每组各40例。观测和比较各组病例治疗前后的颈曲值。结果:3组患者治疗后颈曲值比较,差异均有统计学意义(F=24.08,P<0.001)。经两两比较,治疗后,颈肩背部针刀治疗组颈曲值改善程度优于颈部针刀治疗组(q=5.32,P<0.01),颈部针刀治疗组颈曲值改善程度优于肩背部针刀治疗组(q=4.48,P<0.01)。结论:颈后肌群劳损与颈曲改变存在相关性,足够的颈后肌群力量是维持正常颈曲的重要保障;针刀治疗颈后肌群劳损对颈曲改善具有良好的疗效;单一治疗颈后稳定肌效果优于单一治疗颈后工作肌,二者同时治疗效果均优于单一治疗。

Objective：To make research on cervical flexure change of patients with cervical spondylosis suffering neck muscle strain and effect of aeupotomy treatment. To provide the useful treatment. Methods ： According to anatomic and kinesiologieal characters of neck muscles,to find out the effect of the cervical flexure change suffering from neck muscles strain. A total of 120 patients with cervical spondylosis and cervical flexure change were divided into 3 groups （treatment with acupotomy in the neck group,treatment with aeupotomy in the shoulders and back group, treatment with acupotomy in neck, shoulders and back group） randomly. Every group had 40 patients. Cervical flexure values of groups were compared before and after treatment. Results ：There was significant difference in three groups （ F = 24.08, P 〈 0. 001 ）. After treatment, cervical flexure values＇ improvement degree of treatment with aeupotomy in the neck ,shoulders and back group were better than those of the treatment with acupotomy in the neck group （q = 5.32, .P 〈 0.01 ）. After treatment, cervical flexure values＇ improvement degree of treatment with acupotomy in the neck group were better than that of the treatment with acupotomy in the shoulders and back group（ q =4.48 ,P 〈0. 01 ）. Conclusion ：There is relationship between neck muscle strain and cervical flexure change. Enough power of neck muscles ensures normal cervical flexure value. Aeupotomy makes a good effect on improving the cervical flexure value by treating cervical spondylosis. Effect of treatment to neck stable muscles is better than effect of treatment to neck working muscles. Effect of treatment to both neck stable muscles and neck working muscles is better than effect of treatment to either neck working muscles or neck stable muscles.
