不同产程阶段剖宫产术与母婴并发症发生的相关性研究

Study of the correlation between cesarean section at different stages of labor and maternal and neonatal complications

目的探讨不同产程阶段的剖宫产术与母婴并发症发生的相关性。方法选取2011年6月-2015年6月在该院行剖宫产术的产妇120例为研究对象,并根据其不同产程分为:第一产程剖宫产产妇60例为观察组,第二产程剖宫产产妇60例为对照组。监测比较两组产妇的产后总出血量、手术时间以及术中术后各种并发症发生情况,其新生儿5 min内Apgar综合评分以及各种并发症发生情况,并进行统计分析。结果观察组产妇的产后总出血量、手术时间以及术中术后各种并发症发生率明显低于对照组,差异有统计学意义(P<0.05);观察组新生儿5 min内Apgar综合评分以及各种并发症发生率明显优于对照组,差异有统计学意义(P<0.05)。结论第一产程行剖宫产相对于第二产程行剖宫产,更有利于母婴健康,减少产妇以及新生儿并发症发生,因此,在临床上剖宫产术应选择合适的产程阶段,尽量指导产妇在第一产程行剖宫产,以利于母婴健康。

Objective To explore the correlation between cesarean section at different stages of labor and maternal and neonatal complications. Methods A total of 120 pregnant women receiving cesarean section in this hospital from June 2011 to June 2015 were selected, then they were divided into observation group （the first stage of labor） and control group （the second stage of labor） according to different stages of labor, 60 pregnant women in each group. The amounts of postpartum hemorrhage, operation time, the incidence rates of intraoperarive and postoperative complications, neonatal five-minute Apgar scores, and the incidence rates of neonatal complications in the two groups were monitored and compared, then the data was analyzed statistically. Results The amount of postpartum hemorrhage, operation time, the incidence rates of intraoperative and postoperative complications in observation group were statistically significantly lower than those in control group （P〈0. 05 ） ; neonatal five-minute Apgar score and the incidence rates of neonatal complications in observation group were statistically significantly better than those in control group （ P〈0. 05 ） . Conclusion Compared with the second stage of labor, cesarean section conducted at the first stage of labor is more conducive to maternal and neonatal health, which can reduce the incidence rates of maternal and neonatal complications ; cesarean section should be conducted at appropriate labor stage, the first stage of labor is recommended for maternal and neonatal health.
