摘要
目的研究高血压老年患者采用小剂量右美托咪定对围麻醉期控制血压的效果及安全性。方法临床纳入80例我院2014年8月~2015年9月手术治疗的高血压老年患者作为研究对象,按入院顺序分为两组,各40例。其中40患者采用咪达唑仑静滴作为对照组,另40例患者采用小剂量右美托咪定静滴治疗作为观察组。观察两组患者血流动力学指标、拔管时间、睁眼时间以及不良反应情况。结果观察组患者T_1、T_2、T_3时期与对照组HR、MAP以及SpO_2水平对比,差异有统计学意义,P〈0.05;且观察组T0~T3间HR、MAP以及SpO_2水平波动较稳定。两组患者拔管时间和睁眼时间对比,差异无统计学意义,P〉0.05。两组患者不良反应包括心动过缓、恶心呕吐、寒战以及呼吸抑制等,观察组、对照组发生率分别为10.0%、30.0%,观察组较低,P〈0.05。结论老年高血压患者手术中采用小剂量右美托咪定围麻醉期效果较好,血流波动稳定,不良反应少。
Objective To study the effect and security of low-dose dexmedetomidine in the control of blood pressure of elderly hypertensive patients during perianesthesia. Methods 80 cases of elderly patients with hypertension from August 2014 to September 2015 in our hospital were selected and divided into two groups of 40 cases according to the admission order. 40 cases in the control group were treated with Midazolam static drops,while 40 cases in the observation group were treated with low-dose Demedetomidine static drops. The haemodynamics index,extubation time,awareness time and adverse reaction of patients in both groups were observed. Results There were significant differences in the HR,MAP,and Sp O2 level between two groups in T1,T2,and T3,with the difference statistically significaly,P〈0.05,from T0 to T3,the HR,MAP,and Sp O2 level of the observation group were relatively stable. There was no significant difference in the extubation time and awareness time,with no the difference statistically significaly,P〉0.05. There were some adverse reactions including bradycardia nausea,vomiting,chills,and respiratory depression. The incidence of the observation group and the control group were 10.0% and 30.0%. The observation is significantly lower,P〈0.05. Conclusion The effect of low-dose Dexmedetomidine in the control of blood pressure of elderly hypertensive patients during peri-anesthesia is significant,with stable bloodstream fluctuation and less adverse reactions, which is worth clinical application and promotion.
