磷酸西格列汀联合短期胰岛素强化治疗对初发2型糖尿病患者血清25-羟维生素D及胱抑素C水平的影响 被引量:10

摘要 目的:探讨磷酸西格列汀联合短期胰岛素强化治疗对初发2型糖尿病患者血清25-羟维生素D及胱抑素C水平的影响。方法:选择我院收治的初发2型糖尿病患者70例,根据电脑生成的随机数字表将所有患者随机分为实验组与对照组,每组35例。对照组患者给予磷酸西格列汀进行治疗,实验组患者在对照组的基础上联合短期的胰岛素强化治疗。比较治疗前后两组患者者血清25-羟维生素D(25(OH)D)、胰岛素抵抗指数(HOMA-IR)、胱抑素C(Cys C)、转化生长因子-β1(TGF-β1)、空腹血糖(FBG)及餐后2h血糖(2h BG)水平的变化。结果:与治疗前相比,两组患者治疗后的血清25(OH)D水平均升高,HOMA-IR、Cys C、TGF-β1、FBG及2h BG水平均降低(P<0.05);与对照组相比,实验组患者血清25(OH)D水平较高,HOMA-IR、Cys C、TGF-β1、FBG及2h BG水平较低(P<0.05)。结论:磷酸西格列汀联合短期胰岛素强化治疗能够有效改善初发2型糖尿病患者的胰岛素抵抗、肾功能,并降低血糖。 Objective: To investigate the effect of sitagliptin phosphate combined with short-term intensive insulin treatment on the serum 25- hydroxyvitamin D and cystatin c levels of patients with newly diagnosed type 2 diabetes mellitus. Methods: 70 patients with newly diagnosed type 2 diabetes mellitus from our hospital were selected and randomly divided into the control group and the experimental group with 35 cases in each group. The control group was treated by sitagliptin phosphate alone, while the experimental group was treated on the basis of control group with short term intensive insulin therapy. The serum 25-Dihydroxy vitamin D(25(OH)D),Homeostasis model assessment(HOMA-IR), cystatin C(Cys C), transforming growth factor-β1(TGF-β1), fasting blood-glucose(FBG)and 2-hour Blood Glucose(2h BG) levels were compared between two groups before and after treatment. Results: Compared with before treatment, the serum 25(OH)D level was significantly higher in both groups after treatment(P〈0.05), the HOMA-IR, Cys C, TGF-β1,FBG and 2h BG levels were obviously lower(P〈0.05). Compared with the control group, the serum 25(OH)D level was significantly higher in the experimental group(P〈0.05), the HOMA-IR, Cys C, TGF-β1, FBG and 2h BG levels were lower(P〈0.05). Conclusion:Sitagliptin phosphate combined with short-term intensive insulin treatment could effectively improve the insulin resistance, renal function and reduce the blood glucose in patients with newly diagnosed type 2 diabetes.
