中医辩证施治对阴虚阳亢型高血压病患者血压晨峰及变异性的影响

Effect of traditional Chinese medicine dialectical treatment on morning peak and blood pressure variability of hypertension with yin-deficiency and yang-hyperactivity

目的从血压晨峰及变异性的角度分析中医辨证施治治疗高血压的应用价值。方法 135例阴虚阳亢型高血压患者随机分为2组,对照组常规西药治疗,观察组在对照组的基础上加用中医辨证施治;对比两组的降压疗效和中医症候疗效,并分别于治疗前后采用24 h动态血压监测仪测定血压晨峰和变异性。结果观察组的降压和中医症候方的有效率均显著高于对照组(P<0.05)。治疗前,2组的血压晨峰阳性率和峰值、变异性差异无统计学意义(P>0.05);治疗后,2组的上述指标均有所降低,且观察组降低幅度高于对照组,差异有统计学意义(P<0.05)。结论中医辨证施治辅助常规西药治疗阴虚阳亢型高血压,利于减少血压晨峰及变异性,提高整体疗效。

Objective To analyze the application value of traditional Chinese medicine dialectical treats on hypertension from the angle of morning peak and blood pressure variability （BVP）. Method 135 hypertension patients with yin-deficieney and yang-hyperactivity were divided into 2 groups. The control group were treated with conventional Western medicine while the experimental group were treated with traditional Chinese medicine dialectical treatment in addition to the treatment given to the control group. The antihypertensive efficacy and TCM symptoms curative effect were compared. The morning peak and BVP before and after treatment were detected by ambulatory blood pressure monitoring. Findings The antihypertensive efficacy and TCM symptoms curative effect rate of the experimental group were higher than those of the control group （ P 〉 O. 05 ）. Before treatment, there was no difference in positive rate and levels of morning peak between the two groups（P 〉 0.05）. After treatment, both positive rate and levels of morning peak decreased. The decrease in the experimental group was greater than that of the control group （ P 〈 0.05 ）. Before treatment, there was no significant difference in BVP between groups （P 〉 0. 05 ）. After treatment, diurnal BVP, nightly BVP and 24h BVP all decreased. The decrease in the experimental group was greater than that of the control group （ P 〈0. 05 ）. Conclusion Evidence-based medicine of traditional Chinese medicine auxiliary conventional western medicine can effectively reduce morning peak and BVP. It can improve the overall effect in the treatment of hypertension patients with yin-deficiency and yang-hyperactivity.
