摘要
目的探究碎裂QRS波在急性心肌梗死患者诊断及预后中的应用价值。方法整群选取2014年11月—2016年1月在该院心内科就诊的248例冠心病患者。按是否为AMI分为两组各124例。比较不同心电图指标在AMI中的诊断价值,并记录不同心电图病理波形者发生的并发症及心源性死亡的发生率。结果 f QRS波合并病理性Q波敏感度为36.29%,特异性为96.77%,阳性预测值为91.83%,高于单纯f QRS波、单纯病理性Q波。此外,有f QRS波的患者77例,严重并发症发生率为53.25%,心功能不全发生率为49.35%,死亡发生率为19.48%。与无f QRS波患者存在显著性差异。结论 f QRS波用于AMI早期诊断可靠性高,灵敏度好、特异性强,具有临床推广价值。
Objective To explore the fragmented QRS wave in acute myocardial infarction diagnosis and prognosis of patients with application value. Methods Group selection November 2014 to January 2016 in the Department of Cardiology in our hospital for treatment of 248 cases of coronary heart disease patients. According to whether AMI were divided into two groups, each group of 124 cases. Diagnostic value of different electrocardiographic criteria in patients with acute myocardial infarction(AMI) and record the incidence of different pathologic ECG waveform, complications and cardiac death. Results The f QRS with pathological Q wave sensitivity was 36.29% and specificity was 96.77%, the positive predictive value was91.83%, higher than that of pure f QRS And simple pathological Q wave. In addition, f QRS of 77 patients with, serious complications occur rate is 53.25%, heart function, the incidence of 49.35%, the death rate for 19.48%. and no f QRS patients,there was significant difference. Conclusion The f QRS waves are used for the early diagnosis of AMI reliability high, excellent sensitivity, strong specificity, with clinical value.
