门诊药房药品调剂差错原因分析及对策 被引量:18

摘要 目的分析门诊药房药品调剂差错原因,并提出改进对策。方法回顾性分析医院2013年1月至12月的门诊处方,对其中出现错误的132例处方进行错误原因探讨,提出改进策略,并对2014年1月至2015年12月医院门诊药房药品差错情况进行总结,且与2013年的情况进行对比。结果在2013年门诊药房药品差错中,以门诊药师所致差错为主,其中又以药品多发或漏发(23.48%)、药品剂型及规格等差错(36.36%),以及药品包装或名称相似所致差错(20.45%)最常见;通过改进,2014年1月至2015年12月的门诊药房药品差错发生率显著降低。结论对门诊药房药品常见差错原因进行分析,并有针对性地实施改进对策,可有效提高门诊药房药品配发质量,为患者提供更安全的药学服务,保证医疗工作的顺利进行,对维持良好的医患关系意义重大。 Objective To analyze the error causes of the drug dispensing in the outpatient pharmacy and to promote preventive measures.M ethods Retrospective analysis was applied to analyze the prescriptions in 2013,then the causes of 132 prescriptions with errors were investigated,and the improvement strategies were proposed.The errors in the outpatient pharmacy in 2014 and 2015 were summarized and compared with the ones in 2013.Results The major errors of the outpatient pharmacy drugs in 2013 were outpatient pharmacists errors,in which the drug quantity errors(23.48%),drug dosage forms and specification errors(36.36%) and errors caused by similar packaging or names(20.45%) were the most common.Through the implementation of improvement measures,the drug error rates in the outpatient pharmacy in 2014 and 2015 were significantly reduced.Conclusion The analysis and discussion of outpatient pharmacy error causes and preventive measures can effectively ensure the quality improvement of drug dispensing in the outpatient pharmacy,provide patients with safer pharmacy services,ensure the smooth progress of medical work,which is significant in maintaining a good doctor-patient relationship.
