两种超声检查方法在诊断子宫内膜病变中价值比较

Comparison of Two Methods in the Diagnosis of Endometrial Lesions

目的:探讨经阴道和经腹部彩色多普勒超声在子宫内膜病变诊断中的价值比较。方法:采用随机取样法,选取2012年6月至2015年8月我院接诊的180例子宫内膜病变患者,将所选取的子宫内膜病变患者随机分为2组,每组各90例。观察组采用经阴道彩色多普勒超声的方法检查患者子宫内膜病变情况,对照组则采用腹部彩色多普勒超声的方法检查。最后将两组检查结果与病理诊断结果进行对比分析。结果:观察组的病理诊断结果与对照组结果类似,不具有统计学差异(P>0.05),可进行比较。观察组采用经阴道彩色多普勒超声检查,发现子宫黏膜下肌瘤34例,子宫内膜息肉36例,子宫内膜增生16例,子宫内膜癌4例。与病理诊断结果相比较,符合率分别为94.1%、94.4%、87.5%、75.0%。总符合率为92.2%。对照组经腹部彩色多普勒超声后,发现子宫黏膜下肌瘤35例,子宫内膜息肉31例,子宫内膜增生17例,子宫内膜癌7例。与病理诊断结果相比较,符合率分别为82.9%、77.4%、7.06%、57.1%。总符合率为76.7%。经比较发现,观察组与对照组符合率无明显差异(P>0.05);但观察组的总符合率明显高于对照组,差异具有统计学意义(P<0.05)。结论:采用经阴道彩色多普勒超声的方法诊断子宫内膜病变情况取得了良好的效果,提高了诊断准确率,减少了误诊、漏诊的情况,值得广泛应用。

Objective：To evaluate transvaginal color Doppler ultrasonography in the diagnosis of endometrial lesions.Methods：A total of 180 women with endometrial lesions from June 2012 to August2015in our hospital were enrolled in this study and were randomly and equally divided into conventional ultrasound group and transvaginal ultrasound group according to the methods of examination.The pathological results of all cases were collected.Results：In transvaginal ultrasound group,there were 34 cases of submucosal uterine fibroids,36 cases of endometrial polyps,16 cases of endometrial hyperplasia,and 4cases of endometrial cancer after ultrasound scanning,and the diagnostic accordance rates to pathology were 94.1%,94.4%,87.5%,and 75.0%,respectively.The total accordance rate was 92.2%.While by conventional ultrasound,35 cases of submucosal uterine fibroids,31 cases of endometrial polyps,17 cases of endometrial hyperplasia,and7 cases of endometrial cancer were found respectively,the accordance rates were 82.9%,77.4%,7.06%,and 57.1%,respectively,and the total accordance rate was 76.7%.There is a significant difference in total accordance rate between the two methods（P0.05）,but no statistical difference was found in all the subgroups comparison between the two methods.Conclusion：Transvaginal color Doppler ultrasound is valuable for the diagnosis of patients with endometrial lesions.
