失效模式与效应分析在哈萨克族糖尿病患者胰岛素笔安全注射管理中的应用

Application of Failure Mode and Effect Analysis in the Insulin Pen Safety Injection Management in Kazak Patients with Diabetes

目的分析失效模式与效应分析在哈萨克族糖尿病患者胰岛素笔安全注射管理中的应用方法和成效。方法对该院收治的哈萨克族糖尿病患者的临床资料进行分析,通过失效模式与效应分析理念对患者住院阶段胰岛素笔安全注射的流程进行分析,找到其中可能造成差错的缓解,提出相应的优化与改进措施,并对整改的效果进行分析。结果改进前,用药时间差错或者是漏用的RPN得分为(196.7±35.2)分、抽吸的剂量不准确得分为(312.5±37.9)分、患者的配合度不高得分为(294.7±34.8)分、注射不规范得分为(264.8±40.9)分,改进后下降为(55.8±14.9)分、(62.9±18.5)分、(47.8±11.2)分、和(51.7±14.9)分;不良事件的发生率由改进前的13.33%下降为0,对比前后结果差异有统计学意义(P<0.05)。结论失效模式与效应分析对哈萨克族糖尿病患者胰岛素笔安全注射管理中的应用价值高,能够避免管理缺陷发生,值得推广。

Objective To analyze the application method and effect of failure mode and effect analysis in the insulin pen safety injection management in Kazak patients with diabetes. Methods The clinical data of Kazak patients with diabetes admitted and treated in our hospital were analyzed, and the course of insulin pen safety injection was analyzed through the failure model and effect analysis idea, potential causes of errors were found out, the corresponding optimization and improvement measures were put forwards and the rectification effect was analyzed. Results Before improvements, administration time errors or drain with RPN score（196.7±35.2） points, the suction dose inaccurate score（312.5±37.9） points, with the patient＇s score is not high（294.7 ±34.8） points injection is not standardized score（264.8 ±40.9） points, the improved decreased to（55.8±14.9） points,（62.9±18.5） points,（47.8±11.2） points and（51.7±14.9） points; adverse events 13.33% before the rate improved down to 0, before and after comparison of the results of a statistically significant difference（P〈0.05）.Conclusion The application value of failure mode and effect analysis in the insulin pen safety injection management in Kazak patients with diabetes is high, which can better avoid the occurrence of management defect, and it is worth promotion.
