ER、PR和HER2免疫组化检测在乳腺癌穿刺活检中的应用

Application of ER, PR and HER2 immunohistochemical tests in breast cancer biopsy

目的 研究乳腺癌手术标本与穿刺活检组织中人类表皮生长因子受体-2（HER2）、孕激素受体（PR）、雌激素受体（ER）免疫组化检测的准确性。方法 选取2014年8月~2015年8月间在本院实施乳腺癌根治术患者30例为研究对象,比较手术中切除标本和术前穿刺活检HER2、ER、PR免疫组化检测结果。结果 手术前与手术中ER阳性分别为24例（80.0%）、26例（86.7%）;手术前与手术中PR阳性分别为21例（70.0%）、23例（76.7%）;手术前与手术中的HER2阳性分别为18例（60.0%）、20例（66.7%）。组织学一致性、HER2为83.3%,PR为90.0%,ER为93.3%。PR与ER检测的一致性较高,HER2的一致性则相对较低。结论 PR、ER穿刺活检与术中标本的一致性较高,HER2较低,因此,需综合考虑各方面因素再进行手术方案确定。

Objective To study the accuracy of HER2, PR and ER immunohistochemical tests in the surgical sample and biopsy tissue of breast cancer. Methods A tiotal of 30 patients, treated with radical mastectomy in our hospital from August 2014 to August 2015, were selected as research subjects, and the results of HER2, ER and PR immuno- histochemical test in the surgical resection samples and preoperative biopsy were compared. Results The number of positive cases of ER before the surgery and during the surgery were 24（80.0%） and 26（86.7%） respectively; the number of positive cases of PR before the surgery and during the surgery were 21（70.0%） and 23（76.7%） respectively; the num ber of positive cases of HER2 before the surgery and during the surgery were 18（60.0%） and 20（66.7%） respectively. The histological consistency rates of HER2, PR and ER were 83.3%, 90.0% and 93.3% respectively. The consistency rates of PR and ER were higher, while the consistency rate of HER2 was relatively lower. Conclusion The consistency rate of PR and ER in biopsy and surgical sample is higher, while that of HER2 is lower. Hence, various factors should be put into consideration before the surgical protocol is determined.
